泪道置管防脱落盘的临床应用

摘　　要：目的分析泪道置管防脱落盘是否可以解决泪道置管从内眦处脱出、拔管困难的问题。以及观察置入泪道置管防脱落盘的手术疗效。方法回顾性系列病例研究。选取2020年4月至2020年12月期间在我院收治的泪道阻塞的患者33例(40只眼)。实施鼻内镜下泪道成形+置管术(U型泪道引流管),观察术后泪道置管脱出、拔管困难等情况,分析相关原因及泪道置管防脱落盘的有效性。结果根据术后随访的情况,泪道置管脱出的情况仍有发生,然而放置防脱落盘,泪道置管脱出后需二次手术复位的病例减少,且减少了拔管困难的情况。术后出现鼻腔出血、泪道切割改变,对手术治愈率无影响。结论泪道置管防脱落盘可以防止泪道置管从内眦处脱出过长,限制了脱出的泪道置管受外力牵拉作用被完全拉出;同时,泪道置管防脱落盘防止泪道置管被吸入后鼻腔,有效解决拔管困难的问题。泪道置管防脱落盘具有安全性、有效性,减少了患者的痛苦。Objective To analyze whether the anti-shedding disc of lacrimal duct catheterization can solve the problem of prolapse from the inner canthus and extubation of lacrimal duct catheter,and to observe the surgical effect of inserting lacrimal duct catheter to prevent shedding disc.Methods A retrospective case series study.33 patients(40 eyes)who were diagnosed with lacrimal duct obstruction and treated in our hospital from April 2020 to December 2020 were selected.The operation of lacrimal duct shaping and catheterization(U-shaped lacrimal duct drainage tube)was performed under lacrimal duct endoscope.The prolapse of lacrimal duct catheter and the difficulty of extubation were observed,and the related reasons and the effectiveness of lacrimal duct catheterization were analyzed.Results According to the postoperative follow-up,the prolapse of lacrimal duct catheterization still occurred.However,after the placement of the anti-prolapse disc,the number of cases requiring secondary surgical reduction after lacrimal duct catheterization prolapse decreased,and the difficulty of extubation was reduced.Postoperative nasal bleeding and lacrimal duct cutting changes had no effect on the cure rate of the operation.Conclusions The anti-shedding disc of lacrimal duct catheterization can prevent the lacrimal passage catheterization from protruding too long from the inner canthus,and restrict the prolapse of lacrimal passage catheterization to be pulled out completely by external force,and at the same time,the lacrimal duct catheterization anti-shedding disc can prevent the nasal cavity after lacrimal duct catheterization is inhaled,and effectively solve the problem of extubation.The anti-shedding plate of lacrimal duct catheterization is safe and effective,and reduces the pain of patients.
