摘  要:目的探讨人半乳糖凝集素(galectin)-1、galectin-2在支气管肺炎患儿血清中的表达变化及临床意义,并分析二者相关性。方法选取2019年7月至2020年5月来本院住院的支气管肺炎患儿110例为研究组,根据世界卫生组织(WHO)肺炎及重症肺炎诊断标准分为轻症组和重症组,同时选取53例来本院体检的健康儿童为对照组。收集一般资料,检测T细胞亚群(CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)、CD3^(+)CD4^(+)Th、CD3^(+)CD8^(+)Ts、Th/Ts)、免疫球蛋白(IgM、IgA、IgG)水平,酶联免疫吸附法检测血清中galectin-1、galectin-2表达水平,Pearson法分析galectin-1与galectin-2表达的相关性。利用受试者工作特征曲线(ROC)评价血清galectin-1、galectin-2水平对重症支气管肺炎的诊断价值。结果与对照组比较,研究组血清galectin-1、galectin-2表达水平均明显上调,差异均具有统计学意义(P<0.05),轻症组患儿血清中galectin-1、galectin-2、CD3^(+)CD8^(+)Ts水平均高于对照组,CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)、CD3^(+)CD4^(+)Th、Th/Ts、IgM、IgA、IgG水平均低于对照组(P<0.05);重症组血清中galectin-1、galectin-2、CD3^(+)CD8^(+)Ts水平均高于对照组和轻症组,CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)、CD3^(+)CD4^(+)Th、Th/Ts、IgM、IgA、IgG水平均低于轻症组和对照组(P<0.05)。支气管肺炎患儿血清galectin-1、galectin-2水平均与CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)、CD3^(+)CD4^(+)Th、Th/Ts、IgM、IgA、IgG表达呈负相关(P<0.05),与CD3^(+)CD8^(+)Ts水平呈正相关(P<0.05);支气管肺炎患儿血清中galectin-1和galectin-2表达水平呈正相关(r=0.447,P<0.05);ROC结果显示,血清galectin-1、galectin-2水平预测重症支气管肺炎的曲线下面积(AUC)分别为0.643、0.751,对应的灵敏度分别为62.22%、70.77%,特异性分别为62.20%、80.00%,二者联合预测重症支气管肺炎患儿的AUC为0.787,灵敏度为66.67%,特异性为89.23%。结论galectin-1、galectin-Objective To investigate the expression patterns and clinical significance of human galectin-1 and galectin-2 in serum of children with bronchial pneumonia,and to analyze the correlation between the two.Methods A total of 110 children with bronchial pneumonia who visited our hospital from July,2019 to May,2020 were selected as the research group.According to the World Health Organization(WHO)pneumonia and severe pneumonia diagnostic criteria,these patients were divided into the mild group and severe group.During the same period,53 healthy children who visited our hospital for physical examination were selected as the control group.General data were collected and T cell subsets(CD3^(+),CD4^(+),CD8^(+),CD4^(+)/CD8^(+),CD3^(+)CD4^(+)Th,CD3^(+)CD8^(+)Ts,Th/Ts)and immunoglobulin(IgM,IgA,IgG)levels were detected.The expression levels of galectin-1 and galectin-2 in serum were detected by enzyme-linked immunosorbent assay,and the correlation between galectin-1 and galectin-2 expression was analyzed with Pearson method.The diagnostic value of serum galectin-1 and galectin-2 levels for severe bronchial pneumonia was evaluated by receiver operating characteristic curve(ROC).Results Compared with the control group,serum galectin-1 and galectin-2 expression levels in the study group were significantly up-regulated(P<0.05).The levels of galectin-1,galectin-2,CD3^(+)CD8^(+)Ts in the mild group were higher than those in the control group,while CD3^(+),CD4^(+),CD8^(+),CD4^(+)/CD8^(+),CD3^(+)CD4^(+)Th,Th/Ts,IgM,IgA,IgG levels were lower than the control group(P<0.05);The levels of galectin-1,galectin-2,CD3^(+)CD8^(+)Ts in the severe group were higher than those in the control group and the mild group.The levels of CD3^(+),CD4^(+),CD8^(+),CD4^(+)/CD8^(+),CD3^(+)CD4^(+)Th,Th/Ts,IgM,IgA and IgG were lower than those in the mild group and the control group(P<0.05).The levels of serum galectin-1 and galectin-2 in children with bronchial pneumonia were negatively correlated with the expressions of CD3^(+),CD4^(+),CD8^(+),CD4^(+)/CD8^(+
