摘  要:目的探索γ干扰素(IFN-γ)、糖化血清蛋白(GSP)、糖化血红蛋白(HbA1c)联检对2型糖尿病临床诊断和病情监测中的应用价值。方法选取2020年5月至2021年12月在我院就诊的2型糖尿病306例患者作为观察组,同期进行健康体检的306例志愿者作为对照组。抽取空腹静脉血,检测IFN-γ、GSP、HbA1c水平,详细收集患者临床诊治资料,分析三者与2型糖尿病病情严重程度及预后的关系。结果与健康对照组相比,观察组IFN-γ、GSP、HbA1c水平明显偏高(P<0.05)。本组病例并发症发生率为48.4%,主要并发症为糖尿病肾病和糖尿病视网膜病变,其中并发症组的IFN-γ、GSP、HbA1c水平明显高于无并发症组(P<0.05)。IFN-γ、GSP、HbA1c对2型糖尿病均有诊断学价值(AUC=0.758、0.716、0.637,P<0.05),三者联合检测的灵敏度和特异性分别为0.805和0.604。IFN-γ、GSP、HbA1c检测结果与血糖值呈正相关性(r=0.754、0.728、0.757,P<0.05)。结论IFN-γ、GSP、HbA1c对于2型糖尿病的临床诊断与病情监测具有一定的临床指导价值。Objective To explore The application value of combined interferon-γ(IFN-γ),glycosylated serum protein(GSP)and glycosylated hemoglobin(HbA1c)in clinical diagnosis and condition monitoring of type 2 diabetes mellitus.Methods A total of 306 patients with type 2 diabetes mellitus who were treated in our hospital from May,2020 to December,2021 were selected as the observation group of the study,while 306 patients who received physical examination in our hospital during the same period were selected as the control group.IFN-γ,GSP and HbA1c levels in fasting blood of patients were detected.The clinical diagnosis and treatment data of patients were collected in details to analyze the relationship between the three factors and the severity of type 2 diabetes.Results Compared with the control group,levels of IFN-γ,GSP and HbA1c were significantly higher in the observation group(P<0.05).The incidence of complications in the observation group was 48.4%,and the main complications were diabetic nephropathy and diabetic retinopathy.Levels of IFN-γ,GSP and HbA1c in the complication group were significantly higher than those in the non-complication group(P<0.05).IFN-γ,GSP and HbA1c all showed diagnostic values for type 2 diabetes mellitus(AUC=0.758,0.716,0.637,P<0.05).The sensitivity and specificity of the combined detection of IFN-γ,GSP and HbA1c were 0.805 and 0.604,respectively.Conclusion IFN-γ,GSP and HbA1c have clinical guiding values for clinical diagnosis and monitoring of type 2 diabetes mellitus.
