摘 要:目的探讨乙型肝炎病毒表面抗体(HBsAb)单独阳性及乙型肝炎病毒表面抗原(HBsAg)与HBsAb同时阳性这两种不同乙肝标志物结果模式下,两种不同化学发光法检测系统HBsAb检测结果的一致性及可能影响一致性的原因。方法①选取在A系统行乙型肝炎病毒感染标志物检测样本64例,其中HBsAb单独阳性35例为模式一样本;HBsAg与HBsAb同时阳性29例为模式二样本,所有样本同时在B系统行HBsAb检测并比较A/B系统结果。②模式二样本经聚乙二醇6000(PEG6000)处理后,分别经A/B系统再次检测HBsAb,并比较其结果。结果(1)模式一样本,A/B系统HBsAb阳性符合率为91.4%(32/35)(≥10IU/L),其中10~100IU/L浓度范围符合率为90.9%(20/22);大于100IU/L浓度范围符合率为92.3%(12/13)。A/B系统HBsAb定量检测结果差异有统计学意义[68.01(26.15,270.60)vs 50.65(28.86,246.79),P<0.05]。(2)模式二样本,A/B系统HBsAb阳性符合率为37.9%(11/29)(≥10IU/L),其中10~100IU/L浓度范围符合率为40%(10/25),大于100IU/L浓度范围符合率为25%(1/4)。A/B系统间HBsAb定量检测结果差异有统计学意义[25.45(15.98,66.01)vs 5.81(2.82,22.70),P<0.001]。经PEG处理后,A系统HBsAb检测结果较处理前差异有统计学意义[17.88(8.75,49.61)vs 25.45(15.98,66.01),P<0.001];而B系统两者间差异无统计学意义[8.09(1.93,32.52)vs 5.81(2.82,22.70),P>0.05]。A/B系统结果比较,小于10IU/L浓度范围内符合率为72.7%(8/11),10~100IU/L浓度范围内符合率为43.7%(7/16),大于100IU/L浓度范围内的符合率为50%(1/2)。A/B系统HBsAb定量检测结果差异仍有统计学意义[17.88(8.75,49.61)vs 8.09(1.93,32.52),P<0.001]。结论(1)溯源性不同的A/B检测系统对于单独HBsAb阳性样本结果阳性符合率较高,但定量结果存在一定差异;(2)HBsAg-HBsAb免疫复合物的存在可能是影响A/B系统HBsAb检测结果一致性的重要因素。(3)对于HBsAg和HBsAb同时阳性患者,B系统检测结果可能更能反映个体的真实免疫状�Objective To explore the consistency of HBsAb detection results and the cause of inconsistencies in two different chemiluminescence detection systems under two different hepatitis B markers,namely,HBsAb positive alone and HBsAg positive simultaneously.Methods(1)A total of 64 samples were selected for hepatitis B virus infection marker detection in system A.Of which,35 samples of HBsAb positive alone were defined as mode 1,while 29 samples of HBsAg and HBsAb positive simultaneously were defined as mode 2.All 64 samples were tested again by system B,and the results of A and B were compared.(2)After PEG treatment,HBsAb was detected again by A/B system,and the results were compared.Results(1)The coincidence rate of HBsAb results of mode 1 samples between system A and B were:91.4%(32/35)(≥10IU/L),90.9%(20/22)(between 10-100IU/L)and 92.3%(12/13)(>100IU/L).The results of HBsAb in A/B system were significantly different[68.01(26.15,270.60)vs 50.65(28.86,246.79),P<0.05].(2)In mode 2 samples,the positive coincidence rate of HBsAb in A/B system was 37.9%(11/29)(≥10IU/L),and the coincidence rate was 40%(10/25)(between 10-100IU/L),and 25%(1/4)(>100IU/L).The results of HBsAb of system A were significantly higher than B[25.45(15.98,66.01)vs 5.81(2.82,22.70),P<0.001].After PEG treatment,HBsAb results of system A were significantly decreased[17.88(8.75,49.61)vs 25.45(15.98,66.01),P<0.001].However,the results of system B were not significantly different from those before treatment[8.09(1.93,32.52)vs 5.81(2.82,22.70),P>0.05].The coincidence rate of HBsAb results between A and B system were:72.7%(8/11)(<10IU/L),43.7%(7/16)(between 10-100IU/L)and 50%(1/2)(>100IU/L).There was also significant difference in HBsAb results between system A and B[17.88(8.75,49.61)vs 8.09(1.93,32.52),P<0.001].Conclusion(1)For the samples with HBsAb positive alone,there is a high consistency on the results between the A/B systems,although there is a significant difference in the results due to the difference of traceability.(2)HBsAg-HBsAb immune complex
