从肾失封藏、脾不散精探讨早发性卵巢功能不全的临床诊疗思路

摘　　要：肾为阴阳之根本,天癸之化源,藏五脏六腑之精,精对全身的气血津液具有调控作用;脾气散精,上归于心脉,生化为血,后入冲任二脉为经水,充盈胞宫。经血的化生、运行及贮藏皆依赖肾之封藏、脾之散精。早发性卵巢功能不全临床表现为经水不调,伴烘热汗出、失眠等症状,皆为脏腑失和、气血失调、天癸不充之临床表现。“肾失封藏”为发病之根本,“脾不散精”为发病中心环节;临床治疗应育阴涵阳、培阴育阳、滋肾养血、补肾活血而助肾封藏之功,补脾调食、畅情志、调气机而助脾散精之效。Kidney is the root of yin and yang,the source of Tiangui(reproduction-stimulating essence),and it stores the essence of the five zang-organs and six fu-organs which plays a role in regulating qi,blood and body fluid.The essence distributed by the spleen flows upward to the heart vessels and generates blood.And then,the blood enters the thoroughfare and conception vessels,promoting menstruation and filling the uterus.The transformation,tranportation and storage of menstrual blood all depend on the kidney to store the essence and the spleen to distribute the essence.The symptoms of premature ovarian insufficiency such as menstrual irregularities accompanied by fever,sweating,and insomnia are all clinical manifestations of disharmony between zang-organs and fu-organs,disorders of qi and blood,and insufficiency of Tiangui.Kidney failing to storing the essence is the root of the disease,and spleen failing to storing the essence is the central link of the disease.In clinical treatment,we should help the kidney to store the essence by nourishing yin and moistening yang,cultivating yin and breeding yang,nourishing kidney and blood,tonifying kidney and promoting blood circulation,and help the spleen to distribute the essence by benefiting spleen,adjusting the diet,improving the emotion,and regulating qi movement.
