摘　　要：目的:探讨脊髓血管母细胞瘤的MRI表现。方法:搜集经手术病理证实的脊髓血管母细胞瘤患者8例(11个病灶),分析其临床资料及MRI表现。结果:11个病灶中7个表现为类椭圆形结节,肿瘤所在的节段脊髓增粗,脊髓可因严重受压导致移位、变形;6个病灶T_(1)WI呈稍低信号,T_(2)WI呈高信号,边缘较清晰;1个病灶T_(1)WI呈稍高信号,灶内信号不均匀,增强扫描呈不均匀强化。4个病灶表现为大囊小结节,肿瘤呈囊性,有典型囊壁形成,边缘光整,与脊髓分界清晰,内有壁结节,附壁结节大多位于脊髓中央和背侧,1个囊内有分隔;增强扫描囊内分隔及囊壁不强化(若囊壁由胶原纤维构成多不强化),囊液一般不强化,附壁结节明显强化。11个病灶中5个表面可见粗大的供血动脉和引流静脉;8个内部及周围见增粗、迂曲流空血管影,增强扫描呈蚓状、斑点状高信号强化影,余3个未见流空血管影。8例中5例出现脊髓空洞症,T_(1)WI呈低信号,T_(2)WI呈高信号,增强扫描不强化。结论:脊髓血管母细胞瘤的MRI表现具有一定特征性,MRI对脊髓血管母细胞瘤具有较好的诊断价值,有助于提高其诊断准确性。Objective:To explore and analyze the clinical value of MRI in the diagnosis of spinal cord hemangioblastoma.Methods:8 patients with spinal hemangioblastoma(11 lesions)confirmed by surgery and pathology were collected,and their clinical data and MRI manifestations were analyzed.Results:Seven of the 11 lesions showed oval-like nodules.The spinal cord at the segment where the tumor was located was thickened,and the spinal cord could be displaced or deformed due to severe compression.Six lesions showed a slightly low signal on T_(1)WI and a high signal on T_(2)WI with a clear edge.T_(1)WI of 1 lesion showed a slightly higher signal,uneven intrafocal signal and uneven enhancement.Four lesions showed large cystic and small nodules,and the tumors were cystic,with typical cystic walls,smooth edges,and clear boundaries with the spinal cord.Most of the mural nodules were located in the central and dorsal parts of the spinal cord.On the enhanced scan,there was no enhancement of intracapsular separation,capsular wall or capsular fluid or obvious enhancement of mural nodules.In 5 of the 11 lesions,there were large supplying arteries and draining veins on the surface.Eight lesions showed thickened and tortuous hollow vessel shadows and vermiform and speckled high-signal enhancement,and the other 3 lesions showed no hollow vessel shadow.Five patients showed syringomyelia with low signal on T_(1)WI,high signal on T_(2)WI and no enhancement.Conclusions:MRI of spinal cord hemangioblastoma has certain characteristics.The influence of MRI has better diagnostic value for spinal hemangioblastoma,which helps to improve the accuracy of clinical diagnosis.
