强化VMO训练联合玻璃酸钠治疗CP的疗效观察

摘　　要：目的 观察强化股内斜肌(VMO)训练联合玻璃酸钠关节腔注射治疗髌骨软骨软化症(CP)的疗效。方法 选取CP患者120例,根据不同治疗方式均分为联合组和玻璃酸钠组。玻璃酸钠组患者关节腔注射玻璃酸钠治疗,联合组关节腔注射玻璃酸钠治疗后给予强化VMO训练。比较两组患者治疗前后视觉模拟评分(VAS)、Lysholm膝关节评分、临床疗效、VMO与股外侧肌(VL)活动比率、髌骨肌肉围度及膝关节Q角的变化。结果 联合组临床有效率高于玻璃酸钠组(P<0.05)。治疗后,两组VAS评分、膝关节Q角均低于治疗前,且联合组低于玻璃酸钠组(P<0.05)。治疗后,两组Lysholm膝关节评分、VMO/VL活动比率均较治疗前提高,且联合组高于玻璃酸钠组(P<0.05)。结论 强化VMO训练联合玻璃酸钠关节腔注射治疗可显著缓解CP患者疼痛症状,改善膝关节功能,调节其髌骨肌肉围度和膝关节Q角。Aim To observe the effect of intensive vastus medialis oblique(VMO)training combined with intra-articular injection of sodium hyaluronate in the treatment of patellar chondromalacia(CP).Methods:120 patients with CP were selected and divided into combined group and sodium hyaluronate group according to different treatment meth-ods,with 60 patients in each group.Patients in the sodium hyaluronate group were treated with intra-articular injection of sodium hyaluronate,and patients in the combined group were given intensive VMO training after intra-articular injection of sodium hyaluronate.Visual analogue scale(VAS),Lysholm knee joint score,efficacy index,ratio of VMO to vastus lateralis muscle(VL)activity,patella muscle circumference and knee Q angle were compared between two groups before and after treatment.Results:The clinical effective rate of combined group was higher than that of sodium hyaluronate group(P<0.05).VAS score and knee Q angle of the two groups were lower than those before treatment,and that of the combined group was lower than that of the sodium hyaluronate group(P<0.05).Lysholm knee score and VMO/VL activ-ity ratio patella muscle circumference in the two groups were higher than those before treatment,and those in the combined group were higher than those in the sodium hyaluronate group(P<0.05).Conclusion Intensive VMO training com-bined with intra-articular injection of sodium hyaluronate can significantly relieve pain symptoms of CP patients,improve knee function,and adjust the patella muscle circumference and knee Q angle.
