扩张后胸三角皮瓣带蒂转移修复术在面颈部烧伤后瘢痕患者治疗中的应用意义

摘　　要：目的 探讨扩张后胸三角皮瓣带蒂转移修复术在面颈部烧伤后瘢痕患者治疗中的应用意义。方法 抽取2017年1月-2018年12月我院收治的92例面颈部烧伤后瘢痕患者,随机分成观察组与对照组各46例,对照组采用游离皮片移植术,观察组采用扩张后胸三角皮瓣带蒂转移修复术,观察两组修复效果、修复组织挛缩情况、并发症发生率、总有效率。结果 观察组术后3个月、6个月皮肤色泽、弹性质地评分高于对照组,术后1个月、6个月视觉模拟评分量表(VAS)评分低于对照组(P<0.05);观察组术后3个月、6个月时修复组织挛缩率明显低于对照组(P<0.05);术后,观察组创面感染、血肿、皮肤部分坏死发生率低于对照组,数据差异明显(P<0.05);术后,观察组总有效率(93.48%)明显高于对照组(82.61%),两组数据差异明显(P<0.05)。结论 扩张后胸三角皮瓣带蒂转移修复术可改善面颈部烧伤后瘢痕的修复效果,降低修复组织挛缩率,可显著减少并发症,疗效显著,应用价值高。Objective To investigate clinical application significance of expanded thoracic triangular flap pedicled transfer in treatment of faciocervical burn scar patients.Methods The paper chose 92 patients with scar after faciocervical burn treated in our hospital from January 2017 to December 2018,and divided them into observation group and control group randomly,with 46 cases in each group.Control group was treated with free skin graft,and observation group with pedicled transfer of expanded thoracic triangular flap.Repair effect,repair tissue contracture,complication rate and total effective rate between two groups was observed.Results Scores of skin color and elastic texture in observation group were higher than control group 3 and 6 months after operation,scores of visual analogue scale (VAS) were lower than control group 1 and 6 months after operation,(P<0.05);contracture rate of repaired tissue in observation group was significantly lower than control group at 3 and 6 months after operation (P<0.05).After operation,incidence of wound infection,hematoma and partial skin necrosis in observation group was lower than control group (P<0.05).After operation,total effective rate of observation group (93.48%) was significantly higher than control group (82.61%),with significant difference between two groups (P<0.05).Conclusion Pedicled transfer of expanded thoracic triangular flap can improve repair effect of scar after face and neck burn,reduce contracture rate of repaired tissue,and reduce complications significantly,which has significant curative effect and high application value.
