摘　　要：目的探讨自我感受负担(self-perceived burden,SPB)在中青年脑卒中患者病耻感与生存质量间的中介效应。方法2021年2-7月,采用便利抽样法选取南京市某三级甲等医院收治的253例中青年脑卒中患者为研究对象,采用一般资料调查表、慢性病病耻感量表(stigma scale for chronic illness,SSCI)、自我感受负担量表(self-perceived burden scale,SPBS)、世界卫生组织生存质量测定简表(World Health Organization quality of life-BREF,WHOQOL-BREF)对其进行调查。结果中青年脑卒中患者病耻感总分为(72.31±19.12)分,处于中等以下水平;其病耻感与SPB呈正相关(r=0.66,P<0.05),病耻感、SPB与生存质量呈负相关(r=-0.60、-0.53,均P<0.05)。中介模型显示,SPB在病耻感与生存质量间起部分中介作用,其中介效应占总效应的25.49%。结论中青年脑卒中患者病耻感可通过SPB间接影响其生存质量;医护工作者应采取有效措施降低患者的病耻感和SPB水平,进而改善其生存质量。Objective To explore the mediating effect of self-perceived burden(SPB)between stigma and quality of life among young and middle-aged patients with stroke.Methods Convenience sampling method was adopted to select 253 young and middle-aged patients with stroke in a tertiary hospital in Nanjing City from February to July,2021.The general information questionnaire,Stigma Scale for Chronic Illness(SSCI),Self-Perceived Burden Scale(SPBS),and World Health Organization Quality of Life-BREF(WHOQOL-BREF)were used for investigation.Results The total score of stigma among young and middle-aged patients with stroke was(72.31±19.12),which was at the medium level and below.Stigma was positively correlated with SPB(r=0.66,P<0.05),and stigma and SPB were negatively correlated with quality of life(r=-0.60,-0.53,both P<0.05).The mediating model showed that SPB played a partial mediating effect between stigma and quality of life,accounting for 25.49%of the total effect.Conclusions The stigma of young and middle-aged patients with stroke can indirectly influence their quality of life through their SPB.Medical staff should take effective measures to reduce the patients’stigma and SPB,so as to improve their quality of life.
