摘 要:目的 探讨尿蛋白测定与尿红细胞形态检查对慢性肾脏疾病的筛查价值。方法 将2021年3月-2021年9月在南方科技大学医院确诊的60例慢性肾脏疾病患者。对60例患者采用免疫比浊技术进行尿蛋白测定和尿液进行红细胞计数以及形态检查,根据尿蛋白大小以及单位体积正常与异常红细胞的百分比来判别其患病类型,分析上述两组方法筛选出的疾病类型与已经确诊疾病之间的符合率,根据不同方法检测的符合率高低来判别对慢性肾脏疾病的筛查价值。结果 在14种疾病类型中,经过分析尿蛋白不同类型来判别疾病类型中符合率达到100%的有高血压肾病、急性肾炎、慢性肾炎等一共7种,符合率达到70%~100%以上有慢性肾衰、肾小球肾炎、肾病综合征等一共5种、剩下2种为66.67%,合计符合率达95%。经过分析红细胞形态来判别疾病类型中符合率达到100%的有慢性肾炎、慢性肾衰、紫癜型肾炎等一共7种,符合率达到70%~100%以上有高血压肾病、糖尿病肾炎、肾小球肾炎等一共4种,剩下3种为66.67%,合计符合率达88.33%。且尿蛋白测定与尿红细胞形态检查两种检测方式经过t检验显示,两种方法对慢性肾脏疾病的筛查无明显差异(P>0.05)。结论 尿蛋白测定与尿红细胞形态检查对于慢性肾脏疾病的筛查具有较高的符合率,能够判别其肾脏损伤部位与损伤程度,且两种方法筛选差异不明显,联合使用才能最大限度地提高检测的可靠性。Objective To explore the value of urinary protein determination and urinary erythrocyte morphology in the screening of chronic kidney disease. Methods 60 patients with chronic kidney disease diagnosed in our hospital from March 2021 to September 2021. 60 patients were tested for urinary protein,red blood cell count and morphological examination by immunoturbidimetry. Their disease types were identifed according to the size of urinary protein and the percentage of normal and abnormal red blood cells per unit volume. The coincidence rate between the disease types screened by the above two groups of methods and the confirmed diseases was analyzed, According to the coincidence rate of different methods to judge the screening value of chronic kidney disease. Results Among the 14 disease types, 7were identifed by analyzing the different types of urinary protein, including hypertensive nephropathy,acute nephritis and chronic nephritis, with a coincidence rate of more than 70%~100%, 5 were chronic renal failure, glomerulonephritis and nephrotic syndrome, and the remaining 2 were 66.67%, with a total coincidence rate of 95%.;After analyzing the morphology of red blood cells, 7 of the 100% patients with chronic renal disease, chronic renal failure and purpuric nephritis were found to have a coincidence rate of 70%~100%. The coincidence rate was above 4, including hypertension nephropathy, diabetic nephritis and glomerulonephritis. The remaining 3 were 66.67%, with a total coincidence rate of 88.33%. T-test showed that there was no significant difference between the two methods in the screening of chronic kidney disease(P > 0.05). Conclusion The determination of urinary protein and the morphological examination of urinary red blood cells have a high coincidence rate for the screening of chronic kidney disease, and can distinguish the location and degree of renal injury, and the screening difference between the two methods is not obvious. The combined use can maximize the reliability of the detection.
