摘  要:目的 应用基因芯片技术探讨载脂蛋白E基因多态性与冠心病的严重程度与预后关系。方法 自2019年7月-2020年7月202例心内科住院确诊冠心病患者,所有研究对象测定血TC、TG、HDL-C、LDL-C、CREA水平,并观察GRACE评分、ApoE基因并分型。采用SPSS 23.0软件进行统计分析。结果 各基因表型基因频率分布如下:E2/2型为0.99%,E2/3型为17.32%,E3/3型为67.33%,E2/4型为2.48%,E3/4型为10.89%,E4/4型为0.99%。故ApoE2表型37例,ApoE3表型141例,ApoE4表型24例。三组间的血TC、TG、LDL-C、HDL-C、CREA水平、BMI、性别、家族史,以及高血压病、糖尿病的患病率等无显著性差异。在LDL-C水平上比较,E4组高于E2组,存在统计学差异(P <0.01)。组间比较,E4与E2组相比,E4组分值高于E2组。随访6个月后,各组间MACE事件差异无统计学意义(P <0.05)。结论 冠心病预后与ApoE基因多态性存在一定关系,对其检测在冠心病的防治中有重要临床意义。Objective To investigate the relationship between apolipoprotein E gene polymorphism and the severity and prognosis of coronary heart disease. Methods From July 2019 to July 2020, 202patients with coronary heart disease were hospitalized in cardiology department. The levels of blood TC, TG, HDL-C, LDL-C and crea were measured, grace score and apoE gene were observed and typed.SPSS 23.0 software was used for statistical analysis. Results The distribution of phenotypic gene frequency of each gene was as follows: E2/2 was 0.99%, E2/3 was 17.32%, E3/3 was 67.33%, E2/4 was 2.48%, E3/4 was 10.89%, E4/4 was 0.99%. Therefore, there were 37 cases of apoe2 phenotype, 141 cases of apoE3 phenotype and 24 cases of apoE4 phenotype. There was no significant difference in blood TC, TG, LDL-C, HDL-C, CREA levels, BMI, gender, family history, and the prevalence of hypertension and diabetes between the 3 groups. The LDL-C level in E4 group was higher than that in E2 group(P < 0.01). Compared with E2 group, the score of E4 group was higher than that of E2 group. After 6months of follow-up, there was no significant difference in mace events among the groups(P < 0.05).Conclusion There is a certain relationship between the prognosis of coronary heart disease and apoE gene polymorphism, and its detection has important clinical significance in the prevention and treatment of coronary heart disease.
