摘 要:目的探讨血清25羟基维生素D水平对2型糖尿病肾病患者病情程度及预后的评估价值。方法从2018年2月-2019年2月,抽取2型糖尿病(T2DM)患者50例,对所有患者身高、体重进行测量,并抽取其空腹静脉血测量血脂、血清25羟基维生素D、糖化血红蛋白、肾功能等水平,对其1d内尿蛋白排泄率进行测量,分析糖尿病肾病与血清25羟基维生素D水平间关系。结果5组患者性别、年龄、BMI、吸烟史、TC、TG、HDL-C、LDL-C各项指标相比,差异不具统计学意义(P>0.05);DN组肥胖率、糖尿病病程、高血压史、糖化血红蛋白水平、血肌酐水平、25羟基维生素D水平各项指标均与正常白蛋白尿组有显著差异(P<0.05);各组吸烟史、高血压史、糖化血红蛋白各项指标差异不显著(P>0.05);维生素D缺乏组维生素D水平、BMI、肥胖率、TC、TG、HDL-C、LDL-C、血肌酐、糖尿病肾病发生率、大量蛋白尿率、uAER水平各指标与其他两组相比有显著差异(P<0.05);25羟基维生素D水平与uAER呈负相关性(r=-1.791,P=0.005),且缺乏维生素D可使糖尿病肾病发生率明显升高(P<0.05)。结论2型糖尿病患者多存在血清25羟基维生素D水平偏低现象,且随其水平下降,患者发生DN的风险会相对提高。Objective To investigate the value of serum 25 hydroxyvitamin D level in evaluating the severity and prognosis of type 2 diabetic nephropathy.Methods From February 2018 to February 2019,50 patients with type 2 diabetes mellitus(T2DM)were selected.The height and weight of all patients were measured,and their fasting venous blood was extracted to measure the levels of blood lipid,serum 25 hydroxyvitamin D,glycosylated hemoglobin,renal function,etc.,and their urinary protein excretion rate within one day.The relationship between diabetic nephropathy and serum 25 hydroxyvitamin D level was analyzed.Results There were no significant differences in gender,age,BMI,smoking history,TC,TG,HDL-C and LDL-C between the five groups(P>0.05);there were significant differences in obesity rate,diabetes duration,hyp ertension history,glycosylated hemoglobin level,serum creatinine level and 25 hydroxyvitamin D level between DN group and normal albuminuria group(P<0.05);there were significant differences in smoking history,diabetes duration,hypertension history,glycosylated hemoglobin level,serum creatinine level and 25 hydroxyvitamin D level between DN group and normal albuminuria group(P<0.05).The levels of vitamin D,BMI,obesity rate,TC,TG,HDL-C,LDL-C,serum creatinine,incidence of diabetic nephropathy,massive proteinuria and UAER in vitamin D deficiency group were significantly different from those in the other two groups(P<0.05);the level of 25 hydroxyvitamin D was negatively correlated with UAER(r=-1.791,P<0.05).The incidence of diabetic nephropathy was significantly increased by vitamin D deficiency(P<0.05).Conclusion The level of serum 25 hydroxyvitamin D is low in patients with type 2 diabetes,and the risk of DN will be relatively increased with the decrea se of serum 25 hydroxyvitamin D level.
