上关节突截骨导引椎间孔镜对腰椎间盘突出症患者的影响

摘　　要：目的观察腰椎间盘突出症予以上关节突截骨导引椎间孔镜疗法的临床效果分析。方法选择56例腰椎间盘突出症病人,于2018年10月-2021年9月入院,根据随机双盲法分组,分为研究A组(n=29)、研究B组(n=27),研究A组予以上关节突截骨导引椎间孔镜疗法(SapON),研究B组予以后路椎间盘镜或侧路椎间孔镜术疗法,分析两组的手术时间、镜下操作时间、建立通道时间、透视次数等指标变化;分析两组术前、术后1周、术后3个月的腰部、腿部视觉疼痛模拟评分(VAS)、腰部Oswestry功能障碍指数(ODI)等指标变化;分析两组感染、神经受损、硬膜撕裂伤等不良情况。结果研究A组的手术时间、镜下操作时间、建立通道时间、透视次数等围术期指标显著低于研究B组,差异有统计学意义(P<0.05);两组术后1周、术后3个月腰部、腿部视觉疼痛模拟评分(VAS)、腰部Oswestry功能障碍指数(ODI)均有下降,但与研究B组相比,研究A组的术后1周、术后3个月腰部、腿部VAS、ODI指数评分均显著减低,差异有统计学意义(P<0.05)。结论腰椎间盘突出症予以上关节突截骨导引椎间孔镜术疗法,镜下操作具有较高的安全性,透视次数较少,临床效果明确,值得临床推荐应用。Objective To analyze the clinical effect of superior articular process osteotomy navigation(SapON)in lumbar disc herniation patients.Methods 56 lumbar disc herniation patients treated from October 2018 to September 2021 in our hospital were selected.According to random double-blind method,patients were divided into study group A(n=29)and study group B(n=27).Study group A received SapON;the control group received the microendoscopic discectomy or lateral endoscopic lumbar discectomy.Surgical outcomes were appraised through the operation time,endoscopy operating time,pathway construction time,examination frequency as well as lumbar and leg visual analogue scale(VAS)and lumbar Oswestry disability index(ODI)before surgery,after one week and 3m of surgery,as well as adverse reactions like infection rates,nerve injury and dural laceration.Results Compared with study group B,the operation time,endoscopy operating time,pathway construction time and examination frequency in study group A were remarkably reduced(P<0.05);after one week and 3m of surgery,lumbar and leg VAS and lumbar ODI scores for two groups were declined,but related scores in study group A were even lower than study group B(P<0.05).Conclusion The superior articular process osteotomy navigation can reduce the examination frequency,and increase the operating safety and surgical effects.It is worthy of application and promotion.
