腹腔镜上消化道穿孔修补术用于治疗上消化道溃疡穿孔的临床疗效及安全性观察

摘　　要：目的 探讨腹腔镜上消化道穿孔修补术用于治疗上消化道溃疡穿孔的临床疗效及安全性。方法随机抽取2019年1月-2020年12月在汕头潮南民生医院接受手术治疗的上消化道溃疡穿孔71例患者,采用数字表法,随机分为对照组(n=35)和研究组(n=36),给予对照组患者传统开腹手术治疗,给予研究组患者腹腔镜上消化道穿孔修补术治疗,观察对比两组患者的治疗总有效率、术后并发症发生率以及手术指标。结果 研究组患者治疗总有效率为97.22%,显著高于对照组71.43%(P <0.05)。研究组患者术中出血量为(50.12±10.72)mL、住院总时长为(5.50±2.05)d、手术时为(16.37±3.85)min、及排气时长为(12.85±3.81)h,均显著低于对照组;术后并发症发生率,研究组11.11%低于对照组40.00%,组间对比(P <0.05)。结论 腹腔镜上消化道穿孔修补术治疗上消化道溃疡穿孔的临床疗效显著,手术安全性高,术后并发症低,患者住院时长大幅度缩短,值得在临床上推广。Objective To explore the clinical efficacy and safety of laparoscopic upper digestive tract perforation repair in the treatment of upper digestive tract ulcer perforation. Methods From January 2019 to December 2020, 71 patients with upper digestive tract ulcer perforation treated by surgery in our hospital were randomly divided into control group(n =35) and study group(n =36) by digital table method. The patients in the control group were given traditional open surgery and those in the study group were given laparoscopic upper digestive tract perforation repair. The total effective rate,postoperative complication rate and operation index of the two groups were observed and compared.Results The total effective rate in the study group was 97.22%, which was significantly higher than that in the control group(71.43%, P < 0.05). In the study group, the blood loss during operation was(50.12±10.72)mL, the total hospitalization time was(5.50±2.05) d, the operation time was(16.37±3.85)min and the exhaust time was(12.85±3.81) h, which were significantly lower than those in the control group.The incidence of postoperative complications in the study group was 11.11% lower than that in the control group(40.00%, P <0.05). Conclusion Laparoscopic repair of upper digestive tract perforation is effective in treating upper digestive tract ulcer perforation, with high operation safety, low postoperative complications, and greatly shortened hospital stay, which is worthy of clinical promotion.
