彩色多普勒超声在颈部淋巴结疾病的应用价值

摘　　要：目的 分析彩色多普勒超声在颈部淋巴结疾病的应用价值。方法 选取北京市密云区中医医院2018年6月-2021年6月检查的75例颈部淋巴结肿大患者,均采用彩色多普勒超声检查,根据患者治疗及术后随访结果作为金标准,分析彩色多普勒超声诊断价值。结果 以患者治疗及术后随访结果作为金标准,彩色多普勒超声诊断颈部淋巴结肿大良恶性的灵敏度、特异度及准确性分别为91.18%、98.72%、96.43%。恶性组淋巴结最大长径、最短长径、纵横比<2、形态不规整、淋巴门回声异常、皮质回声异常占比率均高于良性组,血流分型Ⅲ型高于良性组,差异有统计学意义(P<0.05)。恶性组颈部淋巴结超声血流峰值血流速度(PSV)、搏动指数(PI)、阻力指数(RI)均高于良性组,差异有统计学意义(P<0.05)。结论 彩色多普勒超声对颈部淋巴结疾病良恶性判定有一定临床价值,根据超声显示淋巴结纵横比、阻力指数及血流分布等参数,可有鉴别颈部淋巴结性质。Objective To analyze the application value of color Doppler ultrasound in cervical lymph node diseases.Methods 75 patients with cervical lymphadenopathy examined from June 2018 to June 2021 in the Miyun District Hospital of Traditional Chinese Medicine in Beijing were selected.They were all examined by color Doppler ultrasound.The results of treatment and postoperative follow-up were used as the gold standard.Analyze the diagnostic value of color Doppler ultrasound.Results With the results of patient treatment and postoperative follow-up as the gold standard,the sensitivity,specificity and accuracy of color Doppler ultrasound in diagnosing benign and malignant cervical lymphadenopathy were 91.18%,98.72%,and 96.43%,respectively.The maximum long diameter,shortest long diameter,aspect ratio of lymph nodes in the malignant group is less than 2,irregular shape,abnormal lymphatic portal echo,and abnormal cortical echo are all higher than the benign group,blood flow type Ⅲ is higher than that of the benign group,the difference is statistically significant Academic significance (P<0.05).The ultrasound peak blood flow velocity (PSV),pulsatility index (PI) and resistance index (RI) of cervical lymph nodes in the malignant group were higher than those in the benign group,and the difference was statistically significant (P<0.05).Conclusion Color Doppler ultrasound has a certain clinical value in judging benign and malignant cervical lymph node diseases.According to the parameters of lymph node aspect ratio,resistance index and blood flow distribution displayed by ultrasound,it can distinguish the nature of cervical lymph nodes.
