莫达非尼联合BiPAP呼吸机治疗慢性阻塞性肺疾病合并睡眠呼吸暂停综合征的临床疗效

摘　　要：目的 研究对慢性阻塞性肺疾病(COPD)合并睡眠呼吸暂停综合征(OSAS)患者采取莫达非尼联合BiPAP呼吸机的治疗方式,为患者的临床效果带来的影响。方法 选取化州市人民医院2018年10月-2020年12月收治的96例COPD合并OSAS患者为研究对象,根据不同的治疗方式将患者随机等分为对照组和研究组。对照组48名患者采取常规药物联合BiPAP呼吸机治疗,研究组48例患者在此基础上口服莫达非尼进行治疗。观察比较两组患者治疗前后的血氧分压(PaO_(2))、血二氧化碳分压(PaCO_(2))、用力肺活量(FVC)、睡眠量表(ESS)评分及不良反应情况。结果 经常规治疗和联合方式治疗4周后,两组患者的PaO_(2)均有明显升高,PaCO_(2)明显降低(P<0.05),但两组间对比差异不明显(P>0.05)。同时研究组患者的ESS评分为(7.72±1.31)分、FVC为(3.95±0.64)L,对照组患者分别为(10.35±1.52)分、(2.82±0.51)L,组间比较结果显示,研究组患者的睡眠质量(ESS)和用力肺活量(FVC)更优(P<0.05);另一方面,研究组的不良反应发生率为8.33%,对照组的为10.42%,两组数据差异无统计学意义(χ^(2)=2.128,P=0.113)。结论 莫达非尼联合BiPAP呼吸机对于治疗COPD合并OSAS患者具有较好的临床效果,可改善患者的睡眠质量和肺功能及肺通气相关指标,值得广泛推广。Objective To study the clinical effects of modafinil combined with BiPAP ventilator for patients with chronic obstructive pulmonary disease (COPD) combined with sleep apnea syndrome (OSAS).Methods A total of 96 COPD patients with OSAS who were admitted to our hospital from October 2018 to December 2020 were selected as the research subjects,and the patients were randomLy divided into the control group and the study group according to different treatment methods.48 patients in the control group were treated with conventional drugs combined with BiPAP ventilator,and 48 patients in the study group were treated with modafinil orally on this basis.Observe and compare the blood oxygen partial pressure (PaO_(2)),blood carbon dioxide partial pressure (PaCO_(2)),forced vital capacity (FVC),sleep scale (ESS) scores and adverse reactions before and after treatment in the two groups.Results After 4 weeks of conventional treatment and combined treatment,the PaO_(2)of the two groups of patients were significantly increased,and PaCO_(2)was significantly reduced,(P<0.05),but the difference between the two groups was not significant (P>0.05).At the same time,the ESS scores of the patients in the study group were (7.72±1.31) points,the FVC was (3.95±0.64) L,and the patients in the control group were (10.35±1.52) points and (2.82±0.51) L respectively.The results of the comparison between the groups showed that the study The sleep quality (ESS) and forced vital capacity (FVC) of patients in the group were better,(P<0.05);on the other hand,the incidence of adverse reactions in the study group was 8.33%,and that in the control group was 10.42% There was no statistically significant difference between the two groups of data (χ^(2)=2.128,P=0.113).Conclusion Modafinil combined with BiPAP ventilator has a good clinical effect in the treatment of patients with chronic obstructive pulmonary disease complicated with sleep apnea syndrome.It can improve the quality of sleep,lung function and related indicators of lung ventilation in patie
