摘　　要：目的 探讨膀胱功能综合训练应用于老年永久性膀胱造瘘术后长期留置尿管患者的效果。方法 选择2019年2月-2020年5月于佛山市高明区人民医院收治的老年永久性膀胱造瘘术后长期留置尿管患者60例,按计算机随机数字法分为研究组和对照组,各30例。对照组行普通膀胱造瘘管管理,研究组在对照组基础上行膀胱功能综合训练。观察两组出院后1年瘘管堵塞及泌尿系统感染、心理状态、生活质量及并发症发生情况。结果 研究组瘘管堵塞及泌尿系统感染发生率均低于对照组(P<0.05);两组世界卫生组织生活质量评定量表(WHOQOL-100)、生活质量评定量表(JQOLI-74)评分均升高,且研究组高于对照组,焦虑自评量表(SAS)、抑郁自评量表(SDS)评分均下降,且研究组低于对照组(P<0.05);研究组并发症发生率(13.33%)低于对照组(40.00%)(P<0.05)。结论 膀胱功能综合训练应用于老年永久性膀胱造瘘术后长期留置尿管患者,可减少造瘘管堵塞,降低泌尿系统感染和并发症的发生,并改善其心理状态,提高生活质量。Objective To investigate the effect of comprehensive bladder function training on elderly patients with long-term indwelling catheter after permanent cystostomy.Methods 60 elderly patients with long-term indwelling urinary catheter after permanent cystostomy in our hospital from February 2019 to May 2020 were randomLy divided into study group and control group,with 30 cases in each group.The control group was managed by ordinary cystostomy,and the study group received comprehensive training of bladder function on the basis of the control group.The fistula blockage,urinary system infection,psychological status,quality of life and complications were observed in the two groups one year after discharge.Results The incidence of fistula blockage and urinary system infection in the study group were lower than those in the control group (P<0.05);The scores of who quality of life assessment scale (WHOQOL-100) and quality of life assessment scale (JQOLI-74) increased in the two groups,and the scores of the study group were higher than those of the control group,the scores of self rating Anxiety Scale (SAS) and self rating Depression Scale (SDS) decreased,and the scores of the study group were lower than those of the control group (P<0.05).The incidence of complications in the study group (13.33%) was lower than that in the control group (40.00%) (P<0.05).Conclusion The application of comprehensive bladder function training in elderly patients with long-term indwelling urinary catheter after permanent cystostomy can reduce fistula blockage,reduce urinary system infection and complications,improve their psychological state and improve their quality of life.
