全身麻醉联合硬膜外麻醉在妇科腹腔镜手术中的应用分析

摘　　要：目的 分析研究全身麻醉联合硬膜外麻醉在妇科腹腔镜手术中的临床应用效果。方法 选取威海市立医院2019年6月-2020年6月进行妇科腹腔镜手术患者100名。按照数字随机分配法将患者分为观察组(n=50)与对照组(n=50),两组患者在年龄、体重以及ASA等一般性资料方面均无明显的差异(P>0.05)。对照组在手术时采用全身麻醉,观察组在手术时采用全身麻醉联合硬膜外麻醉。对照分析两组患者在进行手术后血流动力学指标、手术时间、苏醒时间以及其子宫的改善等情况。结果 对比T0时的两组心率和SBP以及DBP显示无差异(P>0.05);对照组患者T1时心率和SBP以及DBP显著低于T0,而T3、T4时心率和SBP以及DBP则显著高于T0,差异有统计学意义(P<0.05);观察组T1、T2时心率和SBP以及DBP显著低于T0,差异有统计学意义(P<0.05),而T3、T4时和T0时对比无差异,(P>0.05);两组患者T1时心率和SBP以及DBP对比显示无差异(P>0.05),观察组T2、T3、T4时心率和SBP以及DBP显著低于对照组患者,差异有统计学意义(P<0.05)。观察组患者在手术时间以及苏醒时间上均明显低于对照组(P<0.05),观察组子宫改善优良率显著优于对照组(P<0.05)。结论 全身麻醉联合硬膜外麻醉的应用在妇科腹腔镜手术中,对于手术的效果和患者的身体状况均有积极的意义,其临床应用效果显著,值得进行进一步的推广应用。Objective To analyze clinical effect of combined epidural anesthesia in gynecological surgery.Methods The paper chose 100 patients with gynecological laparoscopic surgery in our hospital from June 2019 to June 2020,and divided them into observation group (n=50) and control group (n=50) according to number random distribution method.There was no significant difference in age,weight and ASA between two groups,(P>0.05).Control group was treated with general anesthesia during operation,and observation group with general anesthesia combined with epidural anesthesia during operation.Hemodynamic indexes,operation time,recovery time and the improvement of uterus between two groups was compared and analyzed.Results There was no difference in heart rate,SBP and DBP between two groups at t0 (P>0.05);heart rate,SBP and DBP at T1 of control group was significantly lower than T0,while heart rate,SBP and DBP at T3 and T4 was significantly higher than t0 (P<0.05);heart rate,SBP and DBP at T1 and T2 in observation group was significantly lower than t0 (P<0.05),there was no difference between T3,T4 and t0 (P>0.05);there was no difference in heart rate,SBP and DBP at T1 between two groups (P>0.05).Heart rate,SBP and DBP at T2,T3 and T4 in observation group was significantly lower than control group (P<0.05).Operation time and recovery time of patients in observation group was significantly shorter than control group (P<0.05),excellent and good rate of uterine improvement in observation group was significantly better than control group (P<0.05).Conclusion General anesthesia combined with epidural anesthesia has positive significance for surgical effect and physical condition of gynecological laparoscopic surgical patients,with remarkable clinical application effect,which is worthy of further promotion and application.
