左卡尼汀联合促红细胞生成素治疗尿毒症透析患者肾性贫血的疗效及对透析中肌肉痉挛发生情况的影响观察

摘　　要：目的观察左卡尼汀联合促红细胞生成素(EPO)治疗尿毒症透析患者肾性贫血的疗效及对透析中肌肉痉挛发生情况的影响。方法48例尿毒症透析肾性贫血患者,随机分成观察组和对照组,每组24例。两组均在保证铁剂、叶酸、维生素B12充足的基础上进行透析,透析后,对照组给予EPO治疗,观察组在对照组基础上加用左卡尼汀注射液治疗。比较两组患者治疗前及治疗3、6个月后红细胞计数(RBC)、血红蛋白(Hb)、红细胞压积(HCT),透析中肌肉痉挛发生情况。结果治疗3个月后,观察组RBC、Hb、HCT高于对照组,但差异无统计学意义(P>0.05);治疗6个月后,观察组患者RBC、Hb、HCT分别为(5.35±0.14)×10^(12)/L、(120.51±10.19)g/L、(35.63±0.36)%,明显高于对照组的(4.39±0.11)×10^(12)/L、(110.07±10.18)g/L、(33.35±0.23)%,差异有统计学意义(P<0.05)。治疗3、6个月后,观察组患者透析中肌肉痉挛发生率分别为8.33%、4.17%,低于对照组的33.33%、25.00%,差异具有统计学意义(P<0.05)。结论左卡尼汀联合EPO治疗尿毒症透析患者肾性贫血效果好,且随着左卡尼汀应用时间的延长,贫血纠正效果越明显;同时,应用左卡尼汀注射液后,透析过程中患者肌肉痉挛的发生率降低,透析充分性好,值得推广应用。Objective To observe the efficacy of levocarnitine combined with erythropoietin(EPO)in the treatment of renal anemia in uremic dialysis patients and its influence on the occurrence of muscle spasm during dialysis.Methods A total of 48 patients with uremic renal anemia were randomly divided into observation group and control group,with 24 cases in each group.Both groups underwent dialysis on the basis of sufficient iron,folic acid and vitamin B12.After dialysis,the control group was treated with EPO,and the observation group was treated with levocarnitine injection on the basis of the control group.The red blood cell count(RBC),hemoglobin(Hb),hematocrit(HCT)before treatment and after 3 and 6 months of treatment,and the occurrence of muscle spasm during dialysis were compared between the two groups.Results After 3 months of treatment,the RBC,Hb and HCT in the observation group were higher than those in the control group,but the difference was not statistically significant(P>0.05).After 6 months of treatment,the RBC,Hb and HCT in the observation group were(5.35±0.14)×10^(12)/L,(120.51±10.19)g/L,and(35.63±0.36)%,which were significantly higher than(4.39±0.11)×10^(12)/L,(110.07±10.18)g/L,and(33.35±0.23)%in the control group,and the differences were statistically significant(P<0.05).After 3 and 6 months of treatment,the incidence of muscle spasm during dialysis in the observation group were 8.33%and 4.17%,respectively,which were lower than 33.33%and 25.00%in the control group,and the differences were statistically significant(P<0.05).Conclusion Levocarnitine combined with erythropoietin has a good effect in the treatment of renal anemia in uremic dialysis patients,and the effect of anemia correction became more obvious as the application time of levocanidine was prolonged;meanwhile,the incidence of muscle spasm in patients during dialysis is reduced after the application of levocanidine injection,and the adequacy of dialysis is good,which is worthy of promotion and application.
