血浆Lp(a)联合EuroSCORE Ⅱ评分预测老年冠心病PCI术后不良心血管事件的价值研究

摘　　要：目的:探讨血浆脂蛋白(a)[Lp(a)]联合欧洲心脏手术风险评估系统(EuroSCORE Ⅱ)评分对老年冠心病(CHD)患者经皮冠状动脉介入治疗(PCI)术后不良心血管事件(MACE)的预测价值。方法:选取116例行PCI治疗的老年CHD患者为研究对象,根据术后1年MACE情况分为MACE组(n=22)和无MACE组(n=94)。比较两组患者一般资料、实验室检测指标及EuroSCORE Ⅱ评分;ROC曲线分析血浆Lp(a)联合EuroSCORE Ⅱ评分对PCI术后发生MACE的预测价值。结果:MACE组患者EuroSCORE II评分、血浆Lp(a)水平高于无MACE组(P<0.05);二者用于预测PCI术后MACE发生的曲线下面积AUC值分别为0.615、0.708,联合预测为0.801,高于各单一指标(P<0.05)。血浆Lp(a)及EuroSCORE Ⅱ评分升高是影响老年CHD患者PCI术后MACE发生的独立危险因素(P<0.05)。结论:联合血浆Lp(a)、EuroSCORE Ⅱ评分对预测老年CHD患者PCI术后MACCE发生有重要价值。Objective:To explore the predictive value of plasma lipoprotein(a)[Lp(a)]combined with scores of European System for Cardiac Operative Risk Evaluation Ⅱ(EuroSCORE Ⅱ)for major adverse cardiac events(MACE)in elderly patients with coronary heart disease(CHD)after percutaneous coronary intervention(PCI).Methods:116 elderly patients with CHD undergoing PCI were selected as the subjects.According to presence or absence of MACE 1 year after surgery,they were divided into MACE group(n=22)and non-MACE group(n=94).The general data,laboratory test indicators and EuroSCORE Ⅱ scores of the two groups were compared.The predictive value of plasma Lp(a)combined with EuroSCORE Ⅱ score for MACE after PCI was analyzed by ROC curves.Results:EuroSCORE Ⅱ score and level of plasma Lp(a)in MACE group were significantly higher than those in non-MACE group(P<0.05).AUC values of the two indexes for predicting MACE after PCI were 0.615 and 0.708,respectively.AUC of combined detection was 0.801,which was greater than that of single index(P<0.05).The increased plasma Lp(a)and EuroSCORE Ⅱ score were independent risk factors of MACE in elderly CHD patients after PCI(P<0.05).Conclusion:The plasma Lp(a)combined with EuroSCORE Ⅱ score is of great predictive value for MACE in elderly CHD patients after PCI.
