温经汤联合中药热奄包对寒凝血瘀型原发性痛经患者疼痛相关因子及子宫动脉血流动力学的影响

摘　　要：目的观察温经汤联合中药热奄包对寒凝血瘀型原发性痛经疼痛相关因子及子宫动脉血流动力学的影响。方法选取2019年2月至2020年5月河北省石家庄市中医院妇一科寒凝血瘀型原发性痛经门诊患者94例,按照随机数字表法分为观察组与对照组,各47例。对照组予布洛芬缓释胶囊口服治疗,观察组予温经汤口服联合中药热奄包热敷治疗。2组均3个月经周期为1个疗程。比较2组治疗前后疼痛相关因子[血清β-内啡肽(β-EP)、前列腺素E_(2)(PGE_(2))、前列腺素F_(2α)(PGF_(2α))、多巴胺(DA)、P物质(SP)、5-羟色胺(5-HT)]水平、子宫动脉血流动力学[子宫动脉搏动指数(API)、阻力指数(RI)、收缩期峰值流速/舒张期峰值流速(S/D)]变化情况。结果2组治疗后与本组治疗前比较β-EP、PGE_(2)水平均升高(P<0.05),PGF_(2α)、DA、SP、5-HT及API、RI、S/D值均降低(P<0.05),且观察组治疗后各项指标改善情况均优于对照组(P<0.05)。结论温经汤联合中药热奄包能够有效减轻寒凝血瘀型原发性痛经患者的疼痛程度,改善疼痛相关因子水平,促进内分泌及子宫动脉血流动力学趋向平衡。Objective To observe the effects of Wenjing Decoction combined with traditional Chinese medicine(TCM)hot package on pain-related factors and uterine artery hemodynamics in patients with primary dysmenorrhea(PD)of cold coagulation and blood stasis type.Methods From February 2019 to May 2020,totally 94 patients with PD(cold coagulation and blood stasis type)admitted to Outpatient of First Department of Gynecology,Shijiazhuang Hospital of Traditional Chinese Medicine were randomly divided into the observation group and the control group,47 cases in each group.The control group was treated with oral ibuprofen sustained-release capsules,and the observation group with Wenjing Decoction combined with TCM hot package.Three menstrual cycle was 1 course,the aim was to compare pain-related factors(serumβ-endorphin[β-EP],prostaglandin E_(2)[PGE_(2)],Prostaglandin F_(2α)[PGF_(2α)],dopamine[DA],substance p[SP],5-hydroxytryptamine[5-HT]),uterine artery hemodynamics(uterine artery pulsatility index[API],resistant index[RI],peak systolic/diastolic flow rate[S/D]).Results After treatment,the levels ofβ-EP and PGE_(2) in groups were increased,and PGF_(2α),DA,SP,5-HT,API,RI and S/D were decreased,and the improvement of which in the observation group were significantly better(P<0.05).Conclusion For patients with PD(cold coagulation and blood stasis type),Wenjing Decoction combined with TCM hot package is effectively capable of relieving pain,improving the level of pain-related factors,and promoting the balance of endocrine and uterine artery hemodynamics.
