温肾助阳汤治疗肾阳虚型慢性前列腺炎合并性功能障碍的疗效及对前列腺液白细胞计数和卵磷脂小体计数的影响

摘　　要：目的观察温肾助阳汤治疗肾阳虚型慢性前列腺炎(CP)合并性功能障碍(SD)的临床疗效及对前列腺液白细胞计数(WBC)和卵磷脂小体(SPL)计数的影响。方法将104例肾阳虚型CP合并SD患者按照随机数字表法分为2组,对照组52例予盐酸坦索罗辛缓释胶囊口服治疗,治疗组52例在对照组治疗基础上加温肾助阳汤,疗程均为8周。比较2组治疗前后美国国立卫生研究院慢性前列腺炎症状指数(NIH-CPSI)、国际勃起功能指数问卷-5(IIEF-5)、前列腺炎相关的性功能障碍评分表(PSFI)、汉密尔顿焦虑量表(HAMA)、汉密尔顿抑郁量表(HAMD)评分,以及前列腺液WBC、SPL计数变化,并统计比较2组总有效率。结果治疗后,2组NIH-CPSI各项评分及总分、PSFI评分、HAMA评分、HAMD评分及前列腺液WBC均较本组治疗前降低(P<0.05),IIEF-5评分和前列腺液SPL计数均较本组治疗前升高(P<0.05),且2组组间比较差异也均有统计学意义(P<0.05)。治疗组总有效率96.15%(50/52),对照组总有效率75.00%(39/52),治疗组总有效率高于对照组(P<0.05)。结论温肾助阳汤治疗肾阳虚型CP合并SD有较好的临床疗效,能降低前列腺液WBC,提高SPL计数。Objective To observe the clinical efficacy of WenshenZhuyang Decoction for chronic prostatitis(CP)complicated with sexual dysfunction(SD)of kidney-yang deficiency type and how to impact on white blood cell count(WBC)and small particle of lecithin(SPL)count in prostatic fluid.Methods Totally 104 patients with CP complicated with SD(kidney-yang deficiency type)were randomly divided into the control group(n=52,oral tamsulosin hydrochloride sustained release capsule)and the treatment group(n=52,WenshenZhuyang Decoction+oral tamsulosin hydrochloride sustained release capsule).Treated for 8 weeks,the aim was to compare the National Institute of Health-Chronic Prostatitis Symptom Index(NIH-CPSI)score,5-item International Index of Erectile Function(IIEF-5),prostatitis-related sexual function index(PSFI),hamilton anxiety scale(HAMA),hamilton depressive scale(HAMD),WBC and SPL count in prostatic fluid;the overall effective rate was assessed.Results After treatment,decreased NIH-CPSI score,PSFI score,HAMA score,HAMD score,WBC in prostatic fluid,and increased IIEF-5 score,SPL count in prostatic fluid were detected in groups(all P<0.05),and with statistically significant difference between groups(P<0.05).The overall effective rate was higher in the treatment group(96.15%[50/52])compared with the control group(75.00%[39/52]),(P<0.05).Conclusion For patients with CP complicated with SD(kidney-yang deficiency type),WenshenZhuyang Decoction can reduce WBC in prostatic fluid,improve SPL count,with better clinical efficacy.
