“缪刺法”治疗带状疱疹后遗神经痛30例临床研究

摘　　要：目的观察“缪刺法”治疗带状疱疹后遗神经痛的临床疗效。方法将60例带状疱疹后遗神经痛患者按照随机数字表法分为2组。治疗组30例予缪刺法治疗,对照组30例予普瑞巴林胶囊治疗,均治疗14 d。比较2组临床疗效及治疗前后疼痛视觉模拟评分(VAS)和血清P物质(SP)、脑源性神经营养因子(BDNF)水平。结果治疗组总有效率93.33%(28/30),对照组总有效率70.00%(21/30),治疗组总有效率高于对照组(P<0.05)。治疗后,2组疼痛VAS均较本组治疗前降低(P<0.05),且治疗组低于对照组(P<0.05)。治疗后,2组患者血清SP水平较本组治疗前降低(P<0.05),BDNF水平均较本组治疗前升高(P<0.05),且治疗组血清SP水平低于对照组(P<0.05),BDNF水平高于对照组(P<0.05)。结论“缪刺法”治疗带状疱疹后遗神经痛能降低血清SP含量,减轻局部炎性反应,同时提高BDNF浓度,保护和修复神经元,减轻疼痛。Objective To observe the clinical effect of contralateral collateral needling for postherpetic neuralgia(PHN).Methods Totally 60 PHN patients were randomly assigned into the treatment group(n=30)and the control group(n=30).The treatment group was treated with contralateral collateral needling,and the control group with pregabalin capsule.A 14-day treatment was performed to compare clinical efficacy,pain visual analogue score(VAS)for pain,serum substance P(SP),brain-derived neurotrophic factor(BDNF).Results The overall effective rate was higher in the treatment group(93.33%[28/30])compared with the control group(70.00%[21/30]),(P<0.05).After treatment,VAS for pain in groups was decreased(P<0.05),the reduction in the treatment group was more prevalent(P<0.05);serum SP in groups was decreased(P<0.05),the treatment group was more common(P<0.05);BDNF levels in groups were increased(P<0.05),which in the treatment group were increased as compared with the control group(P<0.05).Conclusion For PHN patients,contralateral collateral needling is capable of reducing the content of SP,and local inflammatory response,increasing the concentration of BDNF,protecting and repairing neurons,and relieving pain.
