大隐静脉高位结扎联合腔内激光闭合术治疗下肢静脉曲张的临床疗效

摘　　要：目的探讨大隐静脉高位结扎联合腔内激光闭合术治疗下肢静脉曲张的临床疗效。方法选取2020年1月—2021年9月天津市某医院诊治的108例下肢静脉曲张患者为研究对象,采用随机数字表法分为对照组和观察组,每组54例。对照组患者采用大隐静脉高位结扎术治疗,观察组患者在对照组基础上联合腔内激光闭合术治疗,比较2组患者临床疗效、临床手术指标、并发症发生率、静脉曲张评分(AVVSS)及预后效果。结果观察组患者治疗总有效率为98.15%,高于对照组的85.19%,差异有统计学意义(P<0.05)。观察组患者手术时间、住院时间短于对照组,术中出血量小于对照组,差异均有统计学意义(P<0.05)。观察组患者并发症发生率为5.56%,低于对照组的22.22%,差异有统计学意义(P<0.05)。2组患者治疗后AVVSS评分均低于治疗前,且观察组低于对照组,差异均有统计学意义(P<0.05)。观察组患者随访3个月色素沉着减退率、溃疡愈合率均高于对照组,差异有统计学意义(P<0.05);2组患者曲张复发率比较,差异无统计学意义(P>0.05)。结论大隐静脉高位结扎联合腔内激光闭合术治疗下肢静脉曲张效果确切,可降低AVVSS评分,缩短手术时间和住院时间,减少术中出血量,有助于降低并发症发生风险,改善预后。Objective To investigate the clinical efficacy of high ligation of great saphenous vein combined with endovenous laser in the treatment of varicose veins of lower extremities.Methods A total of 108 patients with varicose veins of lower extremities who were diagnosed and treated in a hospital in Tianjin from January 2020 to September 2021 were selected as the subjects and assigned to the control group(n=54)and the observation group(n=54)by the random number table.The patients in the control group received high ligation of the great saphenous vein.The patients in the observation group received endovenous laser treatment in addition to the treatment in the control group.The clinical efficacy,clinical surgical indicators,incidence rate of complications,Aberdeen Varicose Vein Severity Score(AVVSS)and prognosis were compared between the two groups.Results The overall response rate of the observation group was 98.15%,which was higher than that of the control group(85.19%),with statistical significance(P<0.05).The operation time,hospital stay,and intraoperative blood loss in the observation group were shorter than those in the control group,and the differences were statistically significant(P<0.05).The incidence rate of complications in the observation group was 5.56%,which was lower than that in the control group(22.22%),and the difference was statistically significant(P<0.05).The AVVSS of the two groups after treatment were lower than those before treatment,and the observation group was lower than the control group,with statistically significant differences(P<0.05).The hypopigmentation rate and ulcer healing rate in the observation group were higher than those in the control group after a 3-month follow-up,and the differences were statistically significant(P<0.05);the difference in the recurrence rate of varicose veins between the two groups was not statistically significant(P>0.05).Conclusion The high ligation of great saphenous vein combined with endovenous laser treatment is effective in treating varicose veins of l
