摘 要:目的探讨超声诊断肩袖撕裂的灵敏度、特异度、准确率。方法选取2018年9月—2021年1月河南省某医院收治的80例肩袖撕裂疑似患者作为研究对象。所有患者均接受肩部超声扫描检查,以长期随访(≥6个月)或关节镜检查结果为金标准计算超声诊断肩袖撕裂的灵敏度、特异度、准确率,建立接受者操作特征(ROC)曲线分析超声诊断肩袖撕裂的效能,分析肩袖撕裂患者超声特征。结果80例疑似患者中经长期随访或肩关节镜检查确诊肩袖撕裂53例,53例肩袖撕裂患者中经超声检出真阳性42例,真阴性23例,超声诊断肩袖撕裂的准确率为81.25%(65/80)、灵敏度79.25%(42/53)、特异度85.19%(23/27)。ROC曲线显示:曲线下面积(AUC)为0.813。肩袖撕裂患者和非肩袖撕裂患者腱内回声异常发生率(52.83%vs.7.41%)、肩峰撞击综合征发生率(33.96%vs.7.41%)、滑膜囊积液发生率(37.74%vs.70.37%)比较,差异均有统计学意义(P<0.05)。结论超声对肩袖撕裂具有较高诊断价值,临床可通过超声扫描对受检者肩胛下肌腱等部位进行详细观察,腱内回声异常等超声特征可为肩袖撕裂诊断提供参考依据,具有临床推广应用价值。Objective To investigate the sensitivity,specificity and accuracy of ultrasound in diagnosing rotator cuff tear.Methods A total of 80 patients with suspected rotator cuff tears admitted to a hospital in Henan Province from September 2018 to January 2021 were selected as subjects.All patients received shoulder ultrasound scans,and the sensitivity,specificity,and accuracy of ultrasound diagnosis of rotator cuff tears were calculated using long-term follow-up(≥6 months)or arthroscopic findings.The receiver operating characteristic(ROC)curves were established to analyze the efficacy of ultrasound diagnosis of rotator cuff tears and the ultrasound characteristics of patients with this condition.Results Among 80 patients suspected of rotator cuff tears,53 cases of rotator cuff tears were confirmed by long-term follow-up or shoulder arthroscopy,and 42 true-positive and 23 true-negative cases were detected by ultrasound among 53 patients with rotator cuff tears.The accuracy rate of ultrasound diagnosis of rotator cuff tears was 81.25%(65/80),sensitivity 79.25%(42/53),and specificity 85.19%(23/27).The ROC curve showed that the area under the curve(AUC)was 0.813.The differences were statistically significant between patients with rotator cuff tears and those without it in the incidence of abnormal intratendon echo(52.83%vs.7.41%),the incidence of acromion impingement syndrome(33.96%vs.7.41%),and the incidence of synovial bursa effusion(37.74%vs.70.37%)(P<0.05).Conclusion Ultrasound demonstrates high diagnostic value for rotator cuff tears,and the clinical application of ultrasound scanning can provide detailed observation of the subscapularis tendon and other parts of the body,and provide a reference basis for the diagnosis of rotator cuff tears by intratendon echogenic features.These benefits justify a wider application of this technique clinically.
