AMH、IGFs对子宫内膜异位症合并不孕症患者术后妊娠的影响

摘　　要：目的探讨血清抗菌勒管激素(AMH)、胰岛素样生长因子(IGFs)对子宫内膜异位症并发不孕症患者术后妊娠的影响。方法选取2019年3月—2020年3月在广州市某医院进行手术治疗的64例子宫内膜异位症并发不孕症患者为研究对象。根据患者术后1年的自然妊娠情况将其分为2组:妊娠组和非妊娠组,其中妊娠组共41例,非妊娠组共23例。根据妊娠组患者术后自然妊娠时间将其分为3组,术后4个月内为自然妊娠A组(A组),术后5~8个月内为自然妊娠B组(B组),术后9~12个月内为自然妊娠C组(C组),其中A组患者15例,B组患者16例,C组患者10例。比较妊娠组、非妊娠组患者术前、后AMH和IGFs水平;比较A、B、C组患者术前、后AMH、IGFs水平;术前AMH和IGFs水平与术后妊娠时间的相关性分析。结果妊娠组患者术前、后AMH、IGFs水平均高于非妊娠组,差异均具有统计学意义(P<0.05)。3组患者AMH、IGFs水平比较,差异有统计学意义(P<0.05)。随着术后自然妊娠时间的增加,AMH和IGFs水平均逐渐下降,A组患者术前和术后AMH和IGFs水平最高,C组患者术前和术后AMH和IGFs水平最低,差异均具有统计学意义(P<0.05)。术前AMH(r=-0.52,P<0.05)和IGFs(r=-0.46,P<0.05)与术后自然妊娠时间呈负相关关系。结论子宫内膜异位症并发不孕症患者术前AMH、IGFs水平显著影响术后自然妊娠率和自然妊娠时间,AMH、IGFs水平越高,术后自然妊娠时间越短。Objective To investigate the effect of anti-mullerian hormone(AMH)and insulin-like growthfuctors(IGFs)on postoperative pregnancy in patients with endometriosis complicated with infertility.Methods A total of 64 infertile patients with endometriosis who underwent surgery in a hospital in Guangzhou from March 2019 to March 2020 were selected as the study subjects.The patients were assigned to the pregnant group(n=41)and the non-pregnant group(n=23)following the spontaneous pregnancy 1 year after the operation.The patients in the pregnancy group were divided into 3 subgroups following postoperative spontaneous pregnancy time:spontaneous pregnancy group within 4 months after operation(group A),spontaneous pregnancy group within 5-8 months after operation(group B),and spontaneous pregnancy group within 9-12 months after operation(group C),with 15 patients in group A,16 patients in group B and 10 patients in group C.Preoperative and postoperative AMH and IGFs levels were compared between pregnant and non-pregnant patients;preoperative and postoperative AMH and IGFs levels were compared between subgroups A,B,and C.Correlation analysis was performed between preoperative AMH and IGFs levels and postoperative pregnancy time.Results The levels of AMH and IGFs before and after operation in the pregnant group were significantly higher than those in the non-pregnant group(P<0.05).There were significant differences in AMH and IGFs levels among the three subgroups(P<0.05).With the increase in spontaneous pregnancy time after operation,the levels of AMH and IGFs gradually decreased.The levels of AMH and IGFs remained the highest in group A,and the lowest in group C,and the differences were statistically significant(P<0.05).There was a negative correlation between preoperative AMH(r=-0.52,P<0.05)and postoperative IGFs of spontaneous pregnancy time(r=-0.46,P<0.05).Conclusion The preoperative levels of AMH and IGFs in patients with endometriosis complicated with infertility significantly affect the spontaneous pregnancy rate and spo
