育龄期多囊卵巢综合征患者发生子宫内膜癌的危险因素

摘　　要：目的探讨育龄期多囊卵巢综合征(PCOS)患者发生子宫内膜癌的危险因素,并对其进行分析。方法回顾性分析2016年3月至2019年7月于我院就诊的242例育龄期PCOS患者的临床资料,分析其子宫内膜癌发生情况及危险因素。结果242例PCOS患者中有46例发生子宫内膜癌,发生率为19.01%;子宫内膜癌组患者糖尿病史、高血压史、恶性肿瘤家族史、月经初潮年龄(<12岁)、肥胖及高胰岛素血症占比较非子宫内膜癌组患者高(P<0.05);经Logistic回归分析发现,糖尿病史、高血压史、恶性肿瘤家族史、月经初潮年龄(<12岁)、肥胖及高胰岛素血症为导致育龄期PCOS患者发生子宫内膜癌的相关影响因素(OR>1,P<0.05)。结论糖尿病史、高血压史、恶性肿瘤家族史、肥胖等因素均可导致育龄期PCOS患者发生子宫内膜癌。Objective To investigate and analyze the risk factors of endometrial cancer in patients with polycystic ovarian syndrome(PCOS)in child-bearing age.Methods The clinical data of 242 patients with PCOS in child-bearing age who were treated inour hospital from March 2016 to July 2019 were selected to analyzethe occurrenceand risk factors of endometrial cancer.Results There were 46 cases of endometrial cancer in 242 patients with PCOS,and the incidence was 19.01%.The proportion of diabetes history,hypertension history,family history of malignant tumor,menarche age(<12 years old),obesity and hyperinsulinemia in endometrial cancer group were higher than those in non-endometrial cancer group(P<0.05).Logistic regression analysis showed that the diabetes history,hypertension history,family history of malignant tumors,menarche age(<12 years old),obesity and hyperinsulinemia were the influencing factors of endometrial cancer in PCOS patients in child-bearing age(OR>1,P<0.05).Conclusion Diabetes history,hypertension history,family history of malignant tumors and obesity can cause endometrial cancer in patients with PCOS in childbearing age.
