老年下肢骨科手术中应用超声引导下腰丛坐骨神经阻滞麻醉的有效性及对术后VAS的影响

摘　　要：目的分析超声引导下腰丛坐骨神经阻滞麻醉在老年下肢股骨手术中的应用价值。方法选取2019年12月至2021年1月该院收治的下肢骨折患者96例作为研究对象,随机分为研究组和对照组,每组48例。对照组采用常规腰丛坐骨神经阻滞,研究组采用超声引导腰丛坐骨神经阻滞,比较两组患者麻醉效果、阻滞起效、维持时间、疼痛评分及并发症发生情况。结果与对照组比较,研究组患者优良率更高,感觉、运动神经阻滞起效时间均更短,维持时间均更长,疼痛视觉模拟评分(VAS)、并发症发生率均更低,差异均有统计学意义(P<0.05)。结论在老年下肢骨科手术中采取经由超声引导的腰丛坐骨神经阻滞能获得较好的麻醉效果,且麻醉维持时间较长,可有效缓解术后疼痛,值得推广应用。Objective To analyze the value of ultrasounc-guided low back plexus sciatic nerve block anesthesia in the elderly lower extremity femoral surgery.Methods A total of 96 patients with lower extremity fracture admitted to our hospital from December 2019 to January 2021 were selected as the research objects and randomly divided into study group and control group.The control group received conventional lumbar plexus sciatic nerve block,and the study group received ultrasonic guided lumbar plexus sciatic nerve block.The anesthesia effect,block onset,maintenance time,pain score and the occurrence of complications were compared between the two groups.Results Compared with the control group,the rate of good and good was higher in the study group(P<0.05).In the study group,the onset time of sensory block and motor block was shorter,but the maintenance time was longer(P<0.05);VAS score of study group was lower than that of control group(t=10.638,P<0.05).Compared with the control group,the overall incidence of the study group was lower(P<0.05).Conclusion In the elderly lower extremity orthopedic surgery,ultrasound guided lumbar plexus sciatic nerve block can obtain better anesthesia effect,and the anesthesia lasts for a long time,can effectively relieve postoperative pain,it is worth to be popularized and applied.
