摘　　要：目的调查非内分泌科护士胰岛素泵使用的知信行现状和培训需求,为胰岛素泵的系统化培训提供参考依据。方法采用便利整群抽样法,对790名非内分泌科护士采用自行设计的胰岛素泵使用知信行问卷、胰岛素泵使用的培训需求问卷进行调查。结果非内分泌科护士胰岛素泵使用的知识得分为(70.53±15.97)分,态度得分为(90.15±9.92)分,行为得分为(84.99±14.93)分,知识、态度、行为之间呈正相关(P<0.05)。多元线性回归分析显示,是否有内分泌科工作经验、是否操作过胰岛素泵、是否知晓糖尿病相关知识是非内分泌科护士胰岛素泵使用知信行总得分的影响因素(P<0.05)。理论知识培训需求最高的是胰岛素泵治疗的血糖控制目标、胰岛素泵常见问题及原因;操作技能培训需求最高的是输注管路分离、胰岛素泵的日常检查、胰岛素泵常见问题的紧急处置;最需要的培训形式包括院内专题培训、经验分享座谈会。结论非内分泌科护士胰岛素泵使用知识的掌握程度较低,态度较积极,行为有待改善。应建立一套包括三级组织体系、多样化培训形式、个性化培训方案的胰岛素泵培训体系,多方位、多角度地完善胰岛素泵的管理与监督机制。Objective To investigate the status of knowledge,attitude and practice(KAP)and training needs of insulin pump use among nurses in non-endocrinology,so as to provide reference for systematic training of insulin pump.Methods Using convenient cluster sampling method,a total of 790 nurses in non-endocrinology were investigated with self-designed questionnaire on KAP and training needs of insulin pump use and questionnaire on training needs of insulin pump use.Results The scores of knowledge,attitude and practice of non-endocrinology nurses using insulin pump were(70.53±15.97)scores,(90.15±9.92)scores and(84.99±14.93)scores,respectively.There was a positive correlation among knowledge,attitude and practice(P<0.05).Multiple linear regression analysis showed that working experience in the department of endocrinology,operation of insulin pump and awareness of diabetes related knowledge were the influencing factors for the total score of KAP of non-endocrinology nurses(P<0.05).The highest demand for theoretical knowledge training was the blood glucose control target of insulin pump treatment,the common problems of insulin pump and their causes.The highest demand for operation skills training was infusion lines separation,daily inspection of insulin pumps,emergency treatment of common problems of insulin pumps.The most needed forms of training include special in-hospital training and experience-sharing symposiums.Conclusion Nurses in non-endocrinology have a low degree mastery of insulin pump use knowledge,a more positive attitude,and their behavior needs to be improved.We should set up an insulin pump training system including three-level organization system,diversified training forms and personalized training programs,and improve the management and supervision mechanism of insulin pump from multiple perspectives and angles.
