摘 要:目的研究系统性红斑狼疮患者25-羟维生素D[25(OH)D]、生长停滞特异性蛋白6(Gas6)水平及与疾病活动度的关系。方法选取2018年3月—2019年10月本院收治的122例系统性红斑狼疮作为研究组,并根据系统性红斑狼疮疾病活动指数评估患者是否处于活动期。另选取同时期体检健康者87例作为对照组。比较2组及观察组不同疾病活动度患者25(OH)D、Gas6水平,探究25(OH)D、Gas6水平与患者疾病活动度的相关性,分析患者疾病活动度的危险因素。结果研究组25(OH)D水平低于对照组,Gas6水平高于对照组(P<0.01)。活动期患者25(OH)D水平低于稳定期患者,Gas6水平高于稳定期患者(P<0.01)。Spearman相关分析显示,25(OH)D与疾病活动度呈负相关(r=-0.569,P=0.011),Gas6与疾病活动度呈正相关(r=0.639,P=0.018)。多因素Logistic回归分析显示,家族史、25(OH)D及Gas6水平为系统性红斑狼疮患者疾病活动度的独立危险因素(P<0.05,P<0.01)。结论系统性红斑狼疮患者25(OH)D、Gas6水平与患者疾病活动度具有一定相关性,临床通过检测25(OH)D、Gas6水平可评估患者病情。Objective To study the levels of 25 hydroxyvitamin D[25(OH)D]and growth arrest specific protein 6(Gas6)in patients with systemic lupus erythematosus(SLE)and their relationship with disease activity.Methods In total,122 patients with SLE admitted to our hospital from March 2018 to October 2019 were selected as the research group,and whether the patients were in active stage was evaluated according to the criteria of SLE disease activity index.Another 87 healthy physical examinees were selected as the control group.The levels of 25(OH)D and Gas6 in the two groups and in patients with different disease activity levels were compared,and the correlation between the levels of 25(OH)D and Gas6 and the disease activity of patients were explored.The risk factors of disease activity of patients were analyzed.Results The level of 25(OH)D in the research group was lower than that in the control group,while the level of Gas6 was higher than that in the control group(P<0.01).The level of 25(OH)D in patients at active stage was lower than that in patients at stable stage,while the level of Gas6 was higher than that in patients at stable stage(P<0.01).Spearman correlation analysis showed that 25(OH)D was negatively correlated with disease activity(r=-0.569,P=0.011),and Gas6 was positively correlated with disease activity(r=0.639,P=0.018).Multivariate logistic regression analysis showed that family history,25(OH)D and Gas6 levels were independent risk factors for disease activity in patients with SLE(P<0.05,P<0.01).Conclusion The levels of 25(OH)D and Gas6 in patients with SLE have a certain correlation with disease activity.The condition of patients can be evaluated by detecting the levels of 25(OH)D and Gas6.
