摘　　要：目的探讨临床药师在个体化药学服务方面的价值。方法临床药师通过参与1例2型糖尿病肾病合并慢性心力衰竭患者的用药监护,对患者的降糖药物进行优化,给予患者个体化用药指导。结果临床药师参与2型糖尿病肾病合并心力衰竭患者降糖方案的制定,保障了患者用药的安全有效,改善了患者的长期预后。结论临床药师日常工作中应关注合并多种疾病的患者,寻求合适切入点融入临床,提高患者治疗效果。Objective To explore the value of clinical pharmacists in individualized pharmaceutical care.Methods Clinical pharmacists participated in the pharmaceutical care for a patient with type 2 diabetic in nephropathy complicated with chronic heart failure,optimizing the hypoglycemic drugs use and giving individualized medication guidance.Results Clinical pharmacists participated in the formulation of hypoglycemic plan for patients with type 2 diabetic nephropathy complicated with heart failure,ensuring the safety and effectiveness of medication and improving the long-term prognosis of patients.Conclusion Clinical pharmacists should pay attention to patients with multiple diseases in their daily work,seek appropriate entry points to integrate into the clinic at work,and improve the treatment effect on patients.
