从卢云乘医案分析伤寒重症治疗特色

摘　　要：卢云乘《伤寒医验》中有四十余则伤寒病医案,且多为伤寒重症医案,极具研究价值。本文以卢氏医案为主要研究对象,结合书中卢氏对证治方药的理解,从寒邪重症和热邪重症两方面分析卢氏伤寒重症治疗特色,以期为临床应用提供参考。对于寒邪重症,卢氏紧扣表寒重闭和亡阳两大病机,分别予以发散解表,温阳固脱法治疗。对于热邪重症,卢氏抓住热厥和热盛两大病机,以清泄里热、透达郁阳之法治疗热厥重症,而治疗热盛重症则以清热养阴为治疗大法,随证施治。There are more than 40 medical cases in Lu Yuncheng′s Medical Experience of Febrile Disease,and most of them are severe febrile disease cases,which have great research value. This article takes Lu′s medical cases as the main research subjects,combined with Lu′s understanding of syndromes and prescriptions in the book,and analyzes the treatment characteristics of Lu′s severe febrile disease from two aspects. i.e.,severe cold and severe heat,in order to provide reference for clinical application. For severe cold,Lu′s close focus is on the two pathogenesis of exterior cold and Yang extinction,which are treated with two methods:sweating and relieving the surface,warming yang and solidifying the body. For severe heat,Lu seizes the two major pathogenesis of febrile syncope and excess heat,and to treat severe febrile syncope with the method of clearing away internal heat and penetrating and depressing Yang;in the treatment of severe cases of excessive heat,he uses the treatment method of clearing heat and nourishing yin,along with other methods according to the syndrome.
