摘 要:目的探讨电针足三里穴对腹泻型肠易激综合征(diarrhoeal irritable bowel syndrome,IBS-D)模型大鼠结肠组织中蛋白酶激活受体(protease-activated receptor-2,PAR-2)表达的影响。方法在32只雄性SD大鼠中随机选取8只为对照组,8只为对照电针组,余下16只采用慢性束缚应激联合番泻叶灌胃的方法建立IBS-D大鼠模型,待造模成功后随机分为模型组和模型电针组,每组各8只。电针组取足三里穴进行电针干预,留针20 min/d,持续干预7 d。测量大鼠体重增长、腹泻指数、大便Bristol评分,评估直结肠扩张(colorectal distension,CRD)和腹壁撤退反射(abdominal withdrawal reflex,AWR)评分,并运用荧光定量PCR法和Western blot法测定大鼠结肠组织中PAR-2 mRNA及蛋白表达情况。结果与对照组比较,模型组大鼠体重明显降低,腹泻指数与Bristol评分明显升高,CRD容量阈值明显降低,AWR评分升高,PAR-2的mRNA及蛋白相对表达量升高(P<0.01,P<0.05);与模型组比较,模型电针组大鼠体重明显升高,腹泻指数与Bristol评分明显降低,CRD容量阈值明显升高,AWR评分降低,PAR-2的mRNA及蛋白相对表达量降低(P<0.01,P<0.05)。结论电针足三里穴对IBS-D模型大鼠的内脏高敏感性和腹泻情况有明显的改善作用,其作用的机制可能与降低PAR-2的表达水平,降低内脏敏感性有关。Objective:To study the effect of electroacupuncture at Zusanli point on the expression of protease-activated receptor-2(PAR-2)in colon tissue of rats with diarrhea-type irritable bowel syndrome(IBS-D).Methods:Among 32 male SD rats,8 were randomly selected as the control group,8 as the control electroacupuncture group,and the remaining 16 rats were treated with chronic restraint stress combined with senna to establish the IBS-D rat model.After successful modeling,the rats were randomly divided into a model group and a model electroacupuncture group,8 animals each group.In the electroacupuncture group,the electroacupuncture was performed at Zusanli point,retaining for 20 min/d,and the intervention was continued for 7 days.The body weight gain,diarrhea index and excrement Bristol score were measured and the colorectal distension(CRD)and abdominal withdrawal reflex(AWR)scores were evaluated.The fluorescence quantitative PCR and Western blot were used to determine the PAR-2 mRNA and protein expression in colon tissues of the rats.Results:Compared with the control group,the model group had significantly lower body weight,significantly higher diarrhea index and Bristol score,significantly lower CRD volume threshold,higher AWR score,and higher PAR-2 mRNA and protein relative expression(P<0.01,P<0.05).Compared with the model group,the body weight of the rats in the model EA group was significantly increased,the diarrhea index and Bristol score were significantly decreased,the CRD volume threshold was significantly increased,the AWR score was decreased,and the mRNA and protein expression level of PAR-2 were significantly increased(P<0.01,P<0.05).Conclusion:Electroacupuncture at Zusanli point can significantly improve visceral hypersensitivity and diarrhea in IBS-D model rats,and the mechanism may be related to reducing the expression level of PAR-2 and reducing visceral sensitivity.
