摘　　要：目的探讨长链非编码RNA生长停滞特异基因5(LncRNA GAS5)、微小RNA-142-5p(miR-142-5p)在慢性心力衰竭(心衰)患者血清中的表达及临床意义。方法选取2018年2月至2019年2月于济宁医学院附属医院心内科住院治疗的83例慢性心衰患者为慢性心衰组,同期选取健康体检者86例作为健康对照组。采用实时荧光定量PCR(qRT-PCR)法检测血清LncRNA GAS5、miR-142-5p水平,心脏超声测定左室舒张末期内径(LVEDD)、左室收缩末期内径(LVESD)及左心室射血分数(LVEF);分析慢性心衰患者血清LncRNA GAS5、miR-142-5p水平与心功能指标的相关性及二者对慢性心衰的诊断价值;采用Kaplan-Meier法分析血清LncRNA GAS5、miR-142-5p表达与慢性心衰患者预后的关系。结果慢性心衰组血清miR-142-5p水平、LVESD、LVEDD高于健康对照组,血清LncRNA GAS5水平、LVEF低于健康对照组(P<0.05)。慢性心衰患者血清LncRNA GAS5与miR-142-5p水平呈负相关(r=-0.647,P<0.05),且LncRNA GAS5(miR-142-5p)与LVESD、LVEDD呈负(正)相关(P<0.05),与LVEF呈正(负)相关(P<0.05)。血清LncRNA GAS5、miR-142-5p及二者联合诊断慢性心衰的曲线下面积(AUC)分别为0.873、0.896、0.914,特异性分别为82.6%、72.1%、79.1%,敏感度分别为84.3%、91.6%、92.8%。LncRNA GAS5低表达患者2年生存率(57.14%)低于LncRNA GAS5高表达患者2年生存率(85.37%),差异有统计学意义(P<0.05);miR-142-5p高表达患者2年生存率(58.14%)低于miR-142-5p低表达患者2年生存率(85.00%),差异有统计学意义(P<0.05)。结论血清LncRNA GAS5、miR-142-5p表达水平对慢性心衰患者早期诊断及预后评估均有重要作用。Objective To investigate the expression and clinical significance of long non-coding RNA growth arrest specific transcript 5(LncRNA GAS5)and microRNA-142-5p(miR-142-5p)in serum of patients with chronic heart failure.Methods From February 2018 to February 2019,83 patients with chronic heart failure were treated in the Department of Cardiology of our hospital were selected as the chronic heart failure group,and 86 healthy people were selected as the healthy control group.The serum levels of LncRNA GAS5 and miR-142-5p were detected by real-time fluorescence quantitative PCR(qRT-PCR).Left ventricular end-diastolic diameter(LVEDD),left ventricular end-systolic diameter(LVESD)and left ventricular ejection fraction(LVEF)were measured by echocardiography.The correlation between serum LncRNA GAS5,miR-142-5p levels and cardiac function indexes in patients with chronic heart failure and their diagnostic value for chronic heart failure were analyzed.The relationship between the expression of serum LncRNA GAS5 and miR-142-5p and the prognosis of patients with chronic heart failure was analyzed by the Kaplan-Meier method.Results The levels of serum miR-142-5p,LVESD and LVEDD in the chronic heart failure group were higher than those in the healthy control group,while the level of serum LncRNA GAS5 and LVEF in the chronic heart failure group were lower than those in the healthy control group(P<0.05).Serum LncRNA GAS5 was negatively correlated with the level of miR-142-5p in patients with chronic heart failure(r=-0.647,P<0.05),and LncRNA GAS5(miR-142-5p)was negatively(positively)correlated with LVESD and LVEDD(P<0.05),and positively(negatively)correlated with LVEF(P<0.05).The area under the curve(AUC)of serum LncRNA GAS5,miR-142-5p and their combination in the diagnosis of chronic heart failure were 0.873,0.896 and 0.914,respectively,the specificity was 82.6%,72.1%and 79.1%,and the sensitivity was 84.3%,91.6%and 92.8%,respectively.The 2-year survival rate of patients with low expression of LncRNA GAS5(57.14%)was lower than that o
