摘　　要：目的应用瞬时波强(WI)技术评估艾滋病患者颈动脉血管弹性的变化。方法选择长期规律抗病毒治疗的艾滋病患者70例及健康对照65例。测量颈总动脉内中膜厚度(IMT),并进行WI参数测定,比较两组间IMT及WI参数的差异。比较不同年龄分层时WI参数在两组间的差异。结果艾滋病患者的颈总动脉IMT、颈动脉僵硬度(β)、弹性模量(Ep)及脉搏波传导速度(PWVβ)较对照组明显增加,顺应性(AC)减低(P<0.05)。青年艾滋病患者的IMT、β、Ep及PWVβ较青年对照明显增大,AC减低(P<0.05)。中老年艾滋病患者与中老年对照间IMT、β、Ep、PWVβ及AC均无统计学意义(P>0.05)。结论艾滋病患者的颈动脉内中膜增厚、颈动脉硬度增加,发生心血管疾病的风险较人类免疫缺陷病毒(HIV)阴性人群增高,青年患者的表现尤为显著。Objective Applying of wave intensity(WI)technique to assess the changes of carotid artery vascular elasticity in patients with acquired immune deficiency syndrome(AIDS).Methods 70 AIDS patients on long-term regular antiretroviral therapy and 65 healthy controls were selected.The intima-media thickness(IMT)and the WI parameters of the common carotid artery were measured to compare the differences in IMT and WI parameters between the two groups.The differences in WI parameters between the two groups were compared at different age stratification.Results The IMT,carotid artery stiffness(β),elastic modulus(Ep),and pulse wave velocity(PWVβ)were significantly increased and the compliance(AC)was reduced in AIDS patients compared to controls(P<0.05).The IMT,β,Ep,and PWVβwere significantly greater and the AC was decreased in young patients compared to young controls(P<0.05).There were no significant differences in IMT,β,Ep,PWVβ,and AC between AIDS patients and controls in the middle-aged and elderly age stratum(P>0.05).Conclusions AIDS Patients have thickened intima-media and increased carotid stiffness and are at increased risk of cardiovascular disease compared to human immunodeficiency virus(HIV)-negative populations,with particularly significant manifestations in young patients.
