摘 要:目的探讨超声S-Detect技术对直径≤2 cm乳腺小结节良恶性的鉴别诊断价值。方法选取在宜昌市第一人民医院就诊的152例患者共190个乳腺结节,乳腺结节直径≤2 cm,以病理结果为金标准,采用受试者工作特征曲线(ROC)评价常规超声、S-Detect技术、两者联合运用对乳腺小结节的诊断价值。结果190个乳腺结节,经病理确诊良性136个,恶性54个。常规超声诊断乳腺小结节的灵敏度为74.07%、特异度为91.91%、准确度为86.84%;S-Detect技术诊断乳腺小结节的灵敏度为88.89%、特异度为90.44%、准确度为90.00%;联合组诊断乳腺小结节的灵敏度为90.74%、特异度为97.06%、准确度为95.26%。常规超声、S-Detect技术及联合组诊断乳腺小结节良恶性的曲线下面积(AUC)分别为0.830、0.897、0.939,联合组诊断乳腺小结节的AUC明显大于常规超声和S-Detect技术,其差异均有统计学意义(Z=3.494,P=0.005;Z=2.993,P=0.003);常规超声与S-Detect技术间比较差异无统计学意义(Z=1.913,P=0.056)。结论超声S-Detect技术为诊断直径≤2 cm乳腺小结节的良恶性提供了一种新的检查方法,有助于乳腺小结节良恶性的鉴别诊断。Objective To explore the value of ultrasound S-Detect technology in the differential diagnosis of benign and malignant breast nodules with a diameter of≤2 cm.Methods A total of 190 breast nodules in 152 cases treated in our hospital were selected.The diameter of breast nodules was≤2 cm.The pathological results were used as the gold standard,and the receiver operating curve(ROC)was used to evaluate conventional ultrasound,S-detect technology,and the combination of the two method in the diagnosis of small breast nodules.Results Totally 190 breast masses were pathologically examined,136 were benign and 54 were malignant.Conventional ultrasound diagnosis of breast nodules has a sensitivity of 74.07%,a specificity of 91.91%,and an accuracy of 86.84%;S-Detect technology has a sensitivity of 88.89%,a specificity of 90.44%,and an accuracy of 90.00%in the diagnosis of breast nodules;the combined group of the sensitivity in diagnosing small breast nodules was 90.74%,the specificity was 97.06%,and the accuracy was 95.26%.The area under curves(AUCs)of conventional ultrasound,S-Detect technology,and the combined group for the diagnosis of benign and malignant breast nodules were 0.830,0.897,0.939,respectively.The AUC of the combined group for the diagnosis of small breast nodules was significantly greater than that of conventional ultrasound and S-Detect technology,the differences were statistically significant(Z=3.494,P=0.005;Z=2.993,P=0.003);there was no statistically significant difference between conventional ultrasound and S-Detect technology(Z=1.913,P=0.056).Conclusions Ultrasound S-Detect technology provides a new examination method for the diagnosis of benign and malignant breast nodules with diameter≤2 cm,which is helpful for the differential diagnosis of benign and malignant breast nodules.
