摘　　要：目的探讨乳腺癌微钙化与HER2、ER、PR、Ki67的相关性。方法回顾性研究具有病理结果及完整超声图像的乳腺癌患者67例(包含70例病灶),将患者分为两组,有微钙化组和无钙化组,根据二维超声图像,分析其大小及淋巴结转移情况;根据免疫组化结果,分析其基因型和分子分型情况。结果(1)乳腺癌微钙化与大小无关(P>0.05),与淋巴结转移有关(P<0.05);(2)乳腺癌微钙化与HER2基因型相关(P<0.05),与ER、PR、Ki67无关(P>0.05);在HER2阴性的情况下,乳腺癌微钙化与ER、PR同时表现为阳性无关(P>0.05);(3)乳腺癌微钙化与分子分型相关(P<0.05),微钙化在HER2过表达型、Luminal B型乳腺癌中概率较高,Luminal A型乳腺癌往往没有微钙化。结论乳腺癌微钙化与HER2表达相关,且常常伴有淋巴结转移征象。这一结果将为乳腺基因型预判提供一定支持。Objective To investigate the correlation between microcalcification and the expression levels of HER2,ER,PR,Ki67 in breast cancer.Methods All 67 patients(including 70 lesions)with definite pathological findings and complete ultrasound images were retrospectively analyzed and divided into two groups,microcalcification group and non-microcalcification group.According to the ultrasound images,size and lymph node metastasis were analyzed.According to immunohistochemical results,genotypes and molecular typing were analyzed.Results(1)Microcalcification was not related to size(P>0.05),but was related to lymph node metastasis(P<0.05).(2)Microcalcification was correlated with the expression of HER2(P<0.05),but not with the expression of ER,PR,Ki67(P>0.05).In the case of the negative expression rate of HER2,there was no correlation between microcalcification and the positive expression of ER and PR(P>0.05).(3)There was a correlation between microcalcification and molecular types(P<0.05).The probability of microcalcification was higher in HER2-overexpressing type and Luminal B type breast cancer,while there was lower in Luminal A type breast cancer.Conclusions Microcalcification in breast cancer is associated with HER2 expression and lymph node metastasis.This result will provide some support for breast genotype prediction.
