超声技术在抗Jo-1抗体阳性的间质性肺炎病因诊断治疗中的价值

摘　　要：目的探讨弹性成像对于抗Jo-1抗体阳性的间质性肺炎患者病因学诊断。方法利用声脉冲辐射力成像技术对明确抗Jo-1抗体阳性间质性肺炎患者进行股四头肌软硬程度评估,再进行超声引导下股四头肌活检取病理,明确是否存在皮肌炎引起的间质性肺炎的病因学诊断。结果88例抗Jo-1抗体阳性间质性肺炎患者中经病理证实:38例为皮肌炎肌肉组织,50例为正常肌肉组织。因皮肌炎引发的间质性肺炎占所有间质性肺炎的82.6%。46例患者股四头肌VTQ的SWV平均值为(2.01±1.25)m/s。正常股四头肌VTQ的SWV平均值为(2.85±0.22)m/s,皮肌炎股四头肌VTQ的SWV平均值为(1.89±0.35)m/s。皮肌炎性肌肉与正常肌肉VTQ的SWV值比较,P=0.029。结论超声弹性成像对抗Jo-1抗体阳性间质性肺炎的病因学诊断存在一定作用,可以为临床治疗疾病提供依据。Objective To investigate the etiological diagnosis of interstitial pneumonia by acoustic pulse radiation imaging(ARFI)technology in patients with Jo-1 antibody positive.Methods The hardness or softness of the quadriceps muscle in patients with Jo-1 antibody positive interstitial pneumonia were evaluated by using acoustic pulse radiation imaging(ARFI)technology,and then quadriceps femoris biopsy was performed under ultrasound guidance to definite etiological diagnosis.Results Among 88 patients with Jo-1 antibody positive in interstitial pneumonia,38 cases were found to have dermatomyositis muscle tissue,50 cases were found to have normal muscle tissue.Interstitial pneumonia caused by dermatomyositis accounted for 82.6%of all interstitial pneumonia.The mean SWV of VTQ of quadriceps femoris in 46 patients was(2.01±1.25)m/s.The mean SWV of VTQ of the normal quadriceps muscle was(2.85±0.22)m/s.The mean SWV of VTQ of quadriceps femoris in dermatomyositis was(1.89±0.35)m/s.The SWV value of VTQ in dermatomyositis muscle and normal muscle was compared,P=0.029.Conclusions Acoustic pulse radiation imaging(ARFI)technology play a role in the etiological diagnosis of interstitial pneumonia can provide the basis for clinical treatment of disease.
