超声半定量评分结合弹性成像诊断早期肝硬化及其与血清标记物比较

摘　　要：目的探讨超声半定量评分、声触诊弹性成像(STE)、肝纤维化4因子指数(FIB-4)及联合诊断早期肝硬化的价值。方法选取包头医学院第二附属医院慢性乙肝患者77例,根据病理结果分为非肝硬化组和早期肝硬化组,两组均行超声半定量评分、STE、FIB-4检查;采用DeLong检验分析上述指标及其联合对早期肝硬化的诊断效能。结果早期肝硬化组中肝脏硬度值(LSM值)、超声半定量评分及FIB-4高于非肝硬化组,差异有统计学意义;超声半定量评分与STE二者联合(LC)诊断早期肝硬化受试者工作特征曲线下面积(AUC)最高,为0.979,明显优于单独超声半定量评分、STE、FIB-4,差异有统计学意义。结论超声半定量评分与STE二者联合(LC)对早期肝硬化有较高的诊断价值,有助于早期抗病毒治疗。Objective To investigate the diagnostic value of ultrasonic semi-quantitative scoring,sound touch elastography(STE),fibrosis 4 score(FIB-4)and their combination in early stage liver cirrhosis.Methods Seventy-seven patients with chronic hepatitis B diagnosed in our hospital were selected.According to pathological results,the patients were divided into the non-cirrhosis group and the early stage liver cirrhosis group.Both groups were examined by ultrasound semi-quantitative scoring,sound touch elastography(STE),fibrosis 4 score(FIB-4).The Delong test was used to analyze the diagnostic efficacy of the above indicators and their combination in the diagnosis of early stage liver cirrhosis.Results The LSM,ultrasound semi-quantitative score and FIB-4 in the early stage cirrhosis group were higher than those in the non-cirrhosis group,and the difference was statistically significant.The combination of semi-quantitative ultrasound score and STE(linner combination,LC)in the diagnosis of early stage liver cirrhosis has the highest area under the receiver operating characteristic curve(AUC)of 0.979,which is significantly better than the single ultrasound semi-quantitative score,STE,FIB-4,and the difference was statistically significant.Conclusions The combination of semi-quantitative ultrasound score and STE(LC)has a high diagnostic value for early stage liver cirrhosis.This helps early antiviral treatment.
