摘 要:目的探讨应用三维高分辨仿真血流成像产前诊断脐带真结与假结的价值。方法使用二维超声或彩色多普勒超声连续、动态扫查脐带全程,对所有疑似脐带结节进行三维高分辨仿真血流成像。结果超声检查中晚期妊娠并生产的5119个胎儿中共发生脐带真结22例,与三维高分辨仿真血流成像结果符合的脐带真结14例,检出率63.6%,其中单结7例,“8”字结5例,双单结1例,单结及“8”字结共存1例。发生脐带假结15例,三维高分辨仿真血流成像正确识别11例,检出率73.3%。三维高分辨仿真血流成像与二维超声、彩色多普勒超声对脐带真结检出率差异均有统计学意义(P<0.05),彩色多普勒超声与二维超声对脐带真结检出率差异无统计学意义(P>0.05)。结论三维高分辨仿真血流成像技术诊断脐带真结具有较直观的影像学特征,在鉴别真结与假结及其他脐带形态异常上有价值。Objective To evaluate the value of prenatal 3 D HD-live flow in the evaluation of true and false umbilical cord knots.Methods The entire umbilical cord of fetus was examined by two-dimensional ultrasound and color Doppler ultrasonography were used to continuously and dynamically scan the whole umbilical cord,and 3 D HD-live flow was performed for all suspected umbilical cord.Results Among 5119 fetuses in the middle and late stages of pregnancy,22 cases of true umbilical cord nodules were detected by ultrasound and 14 cases were consistent with the results of 3 D HD-live flow,the detection rate was 63.6%.They were 7 cases of single knot,5 cases of“8”knot feature,1 case of double single knot and 1 case of single knot and“8”knot feature.There were 15 cases of false umbilical cord knots,11 cases were correctly identified by 3 D HD-live flow and the detection rate was 73.3%.There were statistically significant differences in the detection rate of true umbilical cord knots between 3 D HD-live flow and two-dimensional ultrasound and color Doppler ultrasonography(P<0.05),and was no significant difference between color Doppler ultrasonography and two-dimensional ultrasound(P>0.05).Conclusions 3 D HD-live flow can stereoscopically and intuitively display the morphological structure of the umbilical cord,It is valuable in distinguishing true and false knots and other abnormalities of umbilical cord.
