摘 要:目的探讨超声在皮肤纤维瘤(DF)与隆突性皮肤纤维肉瘤(DFSP)鉴别诊断中的价值。方法回顾性分析DF和DFSP的超声表现,提取声像图特征并绘制受试者工作特征曲线(ROC)分析其鉴别诊断效能。结果DF和DFSP在性别、部位、范围、最大径、内部回声均匀程度、与周围组织分界、血流信号方面差异有统计学意义(P<0.05)。二者鉴别上最大径诊断效能最佳,其曲线下面积(AUC)为0.88,灵敏度为91.7%,特异度为84.8%,其次是范围及血流信息,其AUC为0.84,灵敏度为83.3%,特异度为84.8%。结论DF和DFSP具有一定的声像图特征,其中最大径、范围、血流信号在二者的鉴别诊断中有较高价值。Objective To investigate the value of ultrasonography in the differential diagnosis of dermatofibroma(DF)and dermatofibrosarcoma protuberans(DFSP).Methods The ultrasonic manifestations of DF and DFSP were analyzed retrospectively,the ultrasonic image features were summarized and the receiver operating characteristic curve(ROC)was drawn to analyze their differential diagnostic efficacy.Results The differences between DF and DFSP are statistical significance in gender,location,range,maximum diameter,internal echo uniformity,boundary with surrounding tissues and blood flow signal(P<0.05).The maximum diameter had the best efficiency in differential diagnosis,the area under the curve(AUC)was 0.88,the sensitivity was 91.7%,and the specificity was 84.8%,followed by range and blood flow information,the AUC was 0.84,the sensitivity was 83.3%,and the specificity was 84.8%.Conclusions Dermatofibroma and dermatofibrosarcoma protuberans have specifical ultrasonic image features,and the maximum diameter,range and blood flow signal have high value in the differential diagnosis.
