摘 要:目的探讨超声造影(contrast-enhanced ultrasound,CEUS)辅助制备下的穿支皮瓣在口腔颌面部组织缺损修复中的应用价值。方法选取15例口腔肿瘤需皮瓣修复的患者,术前用彩色多普勒超声(color Doppler ultrasound,CDUS)和CEUS探测穿支血管,比较分析穿支的数目、管径和收缩期峰值流速(peak systolic velocity,PSV),选择CEUS下优势穿支制备穿支皮瓣,修复口腔颌面部缺损,术后观察皮瓣存活情况并随访远期疗效。结果CDUS共探测到穿支29条,穿支管径:(0.87±0.14)mm,PSV:(8.83±1.01)cm/s;CEUS共探测到穿支37条(P<0.05),其中CDUS所探得的穿支在CEUS下的管径增大到(1.35±0.14)mm(P<0.05),PSV增大到(10.90±1.31)cm/s(P<0.05)。选择CEUS的优势穿支制备皮瓣,15例皮瓣皆成功存活。随访期间患者受区功能基本恢复;供区瘢痕隐蔽,无明显功能障碍。结论相对于CDUS,CEUS可以提高穿支血管的术前筛选和定位效果,在穿支皮瓣修复口腔颌面部组织缺损中具有较高的临床应用价值。Objective To investigate the application value of perforator flap assisted by contrast-enhanced ultrasound(CEUS)in the repair of oral and maxillofacial tissue defects.Methods Fifteen patients with oral tumors requiring flap repair were selected.Perforating vessels were detected by color Doppler ultrasound(CDUS)and contrast-enhanced ultrasound(CEUS)before surgery,and the number,diameter and peak systolic velocity(PSV)of perforating vessels were compared and analyzed.The dominant perforator branch was selected to prepare the perforator flap to repair the oral and maxillofacial defects.The survival of the flap was observed after surgery and the long-term efficacy was followed up.Results A total of 29 perforators were detected by CDUS,with perforations diameter of(0.87±0.14)mm and PSV(8.83±1.01)cm/s.CEUS detected 37 perforators(P<0.05),the diameter of the perforator detected by CDUS under CEUS increased to(1.35±0.14)mm(P<0.05),PSV increased to(10.90±1.31)cm/s(P<0.05).The superior perforator branch of CEUS was selected to prepare flaps,and all 15 flaps survived successfully.During the follow-up period,the function of the patient’s receiving area basically recovered.Donor site scar concealed,no obvious dysfunction.Conclusions Compared with CUDS,CEUS can improve the preoperative screening and location of perforator vessels,and has high clinical application value in repairing oral and maxillofacial tissue defects with perforator flap.
