早孕期胎儿Cantrell五联征的超声表现分析

摘　　要：目的分析早孕期胎儿Cantrell五联征的超声表现。方法回顾性分析在万宁市人民医院收治或从外院转诊的Cantrell五联征早孕期胎儿6例,分析早孕期胎儿Cantrell五联征的超声表现,分析超声诊断与金标准引产产后或尸检金标准确诊的一致性。结果除超声诊断的胎儿异常,金标准引产产后或尸检还检出胎儿1号存在心包缺损、5号存在胸骨异常及膈肌缺损,超声诊断与金标准引产产后或尸检金标准确诊的一致性较好(Kappa=0.818,P<0.05)。6例Cantrell五联征早孕期胎儿超声检出:6例(100.00%)心脏外翻,5例(83.33%)脐膨出或胸腹裂,3例(50.00%)四腔心及三血管切面异常;6例胎儿并发畸形中,2例(33.33%)脊柱侧弯,2例(33.33%)胎儿水肿,1例(16.67%)脑膜脑膨出,1例(16.67%)露脑畸形、鼻骨缺失及单脐动脉;4例(66.66%)颈项透明层(nuchal translucency,NT)增厚。1例(16.67%)早期宫内死亡,5例(83.33%)引产。结论超声检查可检出早孕期胎儿Cantrell五联征,心脏外翻、脐膨出或胸腹裂并伴有畸形是早孕期胎儿Cantrell五联征重要的超声特点。Objective To analyze the ultrasound manifestations of Cantrell pentad in early pregnancy.Methods Six fetuses with Cantrell pentad in early pregnancy who were admitted to our hospital or referred from other hospitals were retrospectively analyzed,and the ultrasound manifestations of Cantrell pentad in the first trimester were analyzed.The consistency between ultrasonography diagnosis and gold standard diagnosis after labor induction or autopsy was analyzed.Results In addition to the fetal abnormalities diagnosed by ultrasound,pericardium defect in fetus 1,sternal abnormality and diaphragm defect in fetus 5 were also detected after gold standard induced labor or autopsy.There was a good consistency between ultrasound diagnosis and gold standard diagnosis after labor induction or autopsy(Kappa=0.818,P<0.05).Ultrasonography of 6 cases of Cantrell pentad in early pregnancy:six cases(100.00%)with cardiac valgus,five cases(83.33%)with omphalocele or thoracogastric fissure,three cases(50.00%)with 4-chamber and 3-chamber heart abnormal vascular section.Among the 6 cases of fetal complications:two cases(33.33%)scoliosis,two cases(33.33%)fetal hydrops,one case(16.67%)meningoencephalocele,one case(16.67%)exoencephaly,nasal bone loss and single umbilical artery,four cases(66.66%)nuchal translucency(NT)thickening.one case(16.67%)died early in utero and five cases(83.33%)were induced labor.Conclusions Ultrasonography can detect Cantrell pentad in early pregnancy.Cardiac valgus,omphalocele or thoracogastric schizophrenia with malformation are important ultrasonographic features of fetal Cantrell’s pentad in early pregnancy.
