摘　　要：目的了解不同人口学特征患者的就医行为特征,分析影响患者就医行为的因素,为相关部门制定政策提供参考。方法采用多阶段单纯随机抽样的方法,在上海市黄浦区的市级、区级医疗机构社区卫生服务中心中,随机抽取878名患者或家属进行问卷调查。结果调查对象两周患病率为17.7%。医疗服务需求居前三位的分别是需要定期体检(72.9%)、简单的保健服务(29.7%)、上门健康咨询和指导(29.6%);患病后首选的医疗机构依次是市级医院(49.5%)、区级医院(31.3%)、社区卫生服务中心(19.2%)。性别、文化程度、家庭人均月收入等是调查对象就医首选医疗机构的影响因素。结论应加大对社区卫生服务中心的软硬件投入,从体制、机制上吸引人才下沉到社区卫生服务中心工作,提升社区卫生综合防治能力;同时针对不同人群特征做好科学就医的宣传工作,引导公众合理地利用医疗卫生资源,精准实施分级诊疗制度,以提高医疗资源的配置效率。Objective To understand the medical seeking behavior of patients with different demographic characteristics,analyze the influencing factors of patients’medical seeking behavior,and provide the reference for government departments to formulate relevant policies.Methods Using the method of multi-stage simple random sampling,878 patients or their family members were randomly selected from the municipal,district and community health care centers in Huangpu District of Shanghai.Results The two-week prevalence rate was 17.7%.The top three medical service demands were regular physical examination(72.9%),simple health care service(29.7%),and on-site health consultation and guidance(29.6%);the top three preferred medical institutions after illness were in municipal hospitals(49.5%),in district hospitals(31.3%),and in community health care centers(19.2%).The influencing factors of the preferred medical institutions were sex,education level,and the average monthly income of families.Conclusion The investment in software and hardware of community health services centre should been increased,attract talents from the system and mechanism to sink into the community health service work,improve the comprehensive prevention and control ability of community health centre,at the same time,doing a good job in the publicity of scientific medical treatment according to the characteristics of different groups,guiding the public to make rational use of medical and health resources,and accurately implementing the hierarchical diagnosis and treatment system should been conducted,so as to improve the efficiency of medical resources allocation.
