前牙牙冠延长术后冠修复应用不同修复材料的临床效果

摘　　要：目的:探讨前牙牙冠延长术后冠修复应用氧化锆材料全瓷修复的临床效果。方法:选择2017年1月~2019年1月于我院口腔科进行前牙牙冠延长术后冠修复的患者166例(214颗患牙),按照随机数字表法分为2组,每组83例。两组均接受前牙牙冠延长术,观察组术后应用氧化锆材料全瓷冠修复,对照组应用钴铬合金材料冠修复。比较两组牙缺损修复情况,两组修复前及修复后12个月龈沟出血指数(SBI)、菌斑指数(PLI)、牙松动度(MD)及龈沟液天冬氨酸转氨酶(AST)、碱性磷酸酶(ALP)含量。结果:观察组患者边缘密合度、牙体折裂、牙体颜色、牙龈健康评价为A级百分率显著高于对照组(P<0.05);修复后12个月观察组SBI、PLI、MD评分均低于对照组(P<0.01),龈沟液AST、ALP含量均低于对照组(P<0.01),差异均有统计学意义。结论:氧化锆全瓷冠修复临床效果显著,可减轻牙周炎症反应,促进牙周组织健康,是前牙牙冠延长术后冠修复理想的材料选择。Objective:To investigate the clinical effect of zirconia all ceramic restoration after anterior crown lengthening.Methods:Totally 166 patients(214 affected teeth)from Jan 2017 to Jan 2019 were divided into two groups,83 cases in each group.Both groups received crown lengthening of anterior teeth,the zirconia all ceramic crowns restoration were used after operation in the observation group,and cobalt chromium alloy crowns were used in the control group.Effect of the restoration were compared betwen two groups.The gingival sulcus bleeding index(SBI),plaque index(PLI),tooth mobility(MD),and content of aspartate aminotransferase(AST),alkaline phosphatase(ALP)in gingival crevicular fluid were compared before and 12 months after restoration.Results:The percentage of A in marginal fit,tooth fracture,tooth color and gingival health of the observation group was significantly higher than that of the control group(P<0.05).The scores of SBI,PLI and MD of the observation group were significantly lower than those of the control group in 12 months after repair(P<0.01),the content of AST,ALP of the control group were significantly lower(P<0.01).Conclusion:The clinical effect of zirconia all ceramic restoration is remarkable,which can reduce the periodontitis reaction and promote the health of periodontal tissue.It is an ideal material selection for crown restoration after crown lengthening of anterior teeth.
