针刺联合规范连续性康复训练治疗脑卒中恢复期患者上肢功能临床观察

摘　　要：目的 观察针刺联合规范连续性康复训练治疗脑卒中恢复期患者临床疗效。方法 将我院脑卒中恢复期患者126例随机分为观察组与对照组各63例,观察组行针灸联合规范连续性康复训练治疗,对照组行常规康复训练治疗,观察两组治疗前后Fugl-Meyer运动功能(FMA)评分、改良Barthel指数(MBI)评分、改良Ashworth痉挛量表(MAS)分级、同型半胱氨酸(Hcy)水平、血管内皮生长因子(VEGF)水平及简明生活质量量表(SF-36)评分。结果 治疗后,两组FMA及MBI评分均较前升高(P<0.05),且观察组的FMA及MBI评分高于对照组(P<0.05);观察组MAS分级显著优于对照组(P<0.05);两组Hcy水平均降低,VEGF水平均升高(P<0.05),观察组Hcy水平低于对照组,VEGF水平高于对照组(P<0.05);两组患者生活质量各维度评分较前升高(P<0.05),且观察组生活质量评分高于对照组(P<0.05)。结论 针刺联合规范连续性康复训练可有效改善脑卒中恢复期患者上肢功能、日常生活能力、上肢肌张力,提升其生活质量。Objective To observe the clinical curative effect of acupuncture combined with standardized and continuous rehabilitation training on patients during stroke recovery. Methods A total of 126 patients during stroke recovery were randomly divided into observation group and control group,63 cases each. Observation group was given acupuncture combined with standardized and continuous rehabilitation training,while control group was given routine rehabilitation training. The scores of FuglMeyer Assessment(FMA) and modified Barthel index(MBI),grading of modified Ashworth Spasticity Scale(MAS),levels of homocysteine(Hcy) and vascular endothelial growth factor(VEGF),and scores of the 36-item short-form health survey(SF-36)before and after treatment in the two groups were observed. Results After treatment,FMA and MBI scores in both groups increased(P<0.05),which were higher in observation group than control group(P<0.05);MAS grading in observation group was significantly better than that in control group(P<0.05);Hcy decreased,while VEGF increased in both groups(P<0.05);Hcy level in observation group was lower than that in control group,while VEGF level was higher than that in control group(P<0.05);scores of quality of life in both groups increased(P<0.05),which were higher in observation group than control group(P<0.05). Conclusion Acupuncture combined with standardized and continuous rehabilitation training can effectively improve upper limb motor function,activities of daily living,upper limb muscular tension in patients during stroke recovery,and improve the quality of life.
