摘  要:目的 调查山东省鲁西南地区手术室护士组织沉默与职业倦怠的现状及相关性,为手术室管理者提供借鉴与指导。方法 采用一般资料调查表、组织沉默量表及职业倦怠量表于2020年5—6月选取山东省鲁西南地区7所三级甲等医院235名手术室护士开展调查。结果 手术室护士组织沉默总分为(47.34±17.14)分,其中默许性沉默维度得分(15.40±5.89)分,防御性沉默得分为(11.50±4.95)分,亲社会性沉默得分为(11.39±4.12)分,漠视性沉默得分为(9.05±5.73)分,默许性沉默得分率最高;职业倦怠综合分为(2.48±0.86)分,其中情绪衰竭维度得分为(16.40±6.96)分,去人格化维度得分为(7.85±4.46)分,个人成就感降低维度得分为(24.44±7.11)分,去人格化得分率最高。手术室护士职业倦怠综合得分与组织沉默总得分及各维度得分均呈正相关(r=0.539~0.846,均P<0.05);漠视性沉默、默许性沉默、亲社会性沉默及个人职称是影响手术室护士职业倦怠的主要因素。结论 山东省鲁西南地区手术室护士普遍存在中等水平的组织沉默行为及轻中度职业倦怠,漠视性沉默、默许性沉默、亲社会性沉默及职称是引起手术室护士职业倦怠的主要影响因素。建议今后重视手术室护士层级管理,关注手术室护士情感需求,创建良好工作氛围。Objective To investigate the current situation and correlation of organizational silence and job burnout among operating room nurses in the southwestern Shandong Province,provide reference and guidance for operating room managers. Methods The general information questionnaire,organizational silence scale,and job burnout scale were used to investigate 235 operating room nurses from seven grade-A tertiary hospitals in southwestern Shandong Province from May to June 2020. Results The total score for organizational silence of operating room nurses was(47.34±17.14)points,of which the dimension of tacit silence was scored(15.40±5.89)points,the score of defensive silence was(11.50±4.95)points,and the score of prosocial silence was(11.39±4.12)points,the score of indifferent silence was(9.05 ±5.73)points,and the scoring rate of tacit silence was the highest. The comprehensive score of job burnout was(2.48±0.86)points,of which the score of emotional exhaustion was(16.40±6.96)points,the score of depersonalization dimension was(7.85 ±4.46)points,the score of reduced personal accomplishment dimension was(24.44 ±7.11)points,and the scoring rate of depersonalization was the highest. The comprehensive score of job burnout in the operating room was positively correlated with the total score of organizational silence and the scores of each dimension(r=0.539-0.846,all P<0.05). The indifferent silence,tacit silence,prosocial silence and personal title were the main factors that affect the job burnout of nurses in the operating room. Conclusions The operating room nurses in southwest Shandong Province generally have moderate organizational silence behaviors and mild to moderate job burnout,and indifferent silence,tacit silence,prosocial silence and job title are the main influencing factors that cause job burnout among operating room nurses. It is advisable to pay attention to the hierarchical management of nurses in the operating room,focus on the emotional needs of nurses in the operating room,and create a good working atm
