深麻醉下插管患儿拔管的研究进展

摘　　要：随着小儿手术种类以及数量的增加,接受全身麻醉气管插管的患儿数量逐年增加,拔管有关并发症发生风险也随之升高,临床医师也越来越关注气管拔管方式以及时机。就小儿全身麻醉后气管拔管的时机来看,可分为清醒状态与深麻醉状态。拔管时机的选择十分关键,时机不当可能引发严重呼吸道并发症,甚至对患儿的生命安全造成威胁。在临床实施小儿麻醉过程中,既要为患儿的生命安全提供积极保障,也要保证患儿能够于舒适环境下恢复意识。当前临床大量研究结果均证实了深麻醉下气管拔管的优势,如降低患儿呼吸道并发症与躁动发生风险、控制机体血流动力学波动情况、有效预防应激反应等,能够使麻醉苏醒阶段的患儿保持舒适状态,对患儿术后恢复具有积极促进意义。本文将围绕小儿深麻醉下气管拔管方式以及用药进行综述,为临床小儿深麻醉下气管拔管提供参考方案。With the increase of the types and number of pediatric operations,the number of children undergoing tracheal intubation under general anesthesia increases year by year,and the risk of complications related to extubation also increases.Clinicians pay more and more attention to the mode and timing of tracheal extubation.The timing of tracheal extubation after general anesthesia in children can be divided into conscious state and deep anesthesia state.The timing of extubation is very important.Improper timing may lead to serious respiratory complications,and even threaten the life safety of children.In the clinical implementation of pediatric anesthesia,we should not only provide positive guarantee for the life safety of children,but also ensure that children can restore consciousness in a comfortable environment.At present,a large number of clinical research results have confirmed the advantages of tracheal extubation under deep anesthesia,such as reducing the risk of respiratory complications and agitation,controlling the fluctuation of body hemodynamics,effectively preventing stress response,etc.,which can keep children in a comfortable state in the recovery stage of anesthesia,and has a positive significance in promoting children’s postoperative recovery.This paper will review the methods and drugs of tracheal extubation under deep anesthesia in children,so as to provide the reference scheme for clinical tracheal extubation under deep anesthesia in children.
