中西医药物联合治疗IgA肾病的研究进展

摘　　要：IgA肾病是一种常见的原发性肾小球疾病,是肾小球源性血尿常见的病因,该疾病主要的临床表现为肉眼血尿或镜下血尿,同时伴有不同程度的蛋白尿,部分患者逐渐进展为终末期肾脏病。中西医在治疗IgA肾病方面各具优势,目前西医仍然以肾素-血管紧张素-醛固酮系统抑制剂、糖皮质激素以及免疫抑制为主要治疗手段,需要严格地掌握药物的适应证与禁忌证,同时密切监测药物不良反应的发生。中医治疗强调运用整体观念对患者进行全身调整治疗,中西医药物联合治疗IgA肾病能够优势互补,在一定程度上减少药物不良反应的发生,在临床治疗过程中得到了广泛的应用。近年来,在国内有许多中西医药物联合治疗IgA肾病的临床研究在不断进行。本文通过收集和总结文献,对IgA肾病中西医联合治疗方法进行综述,以期为临床诊疗提供一定参考,也为今后系统深入的研究奠定基础。IgA nephropathy is a common primary glomerular disease.It is the common cause of glomerular hematuria.The main clinical manifestation of the disease is macroscopic hematuria or microscopic hematuria,accompanied by varying degrees of proteinuria.Some patients gradually progress to end-stage renal disease.Traditional Chinese medicine and Western medicine have their own advantages in the treatment of IgA nephropathy.At present,western medicine still takes renin-angiotensin-aldosterone system inhibitors,glucocorticoids and immunosuppression as the main treatment methods.It is necessary to strictly grasp the indications and contraindications of drugs,and closely monitor the occurrence of drug adverse reactions.Traditional Chinese medicine treatment emphasizes the use of the overall concept to adjust the whole body of patients.The combination of traditional Chinese and Western medicine in the treatment of IgA nephropathy can complement each other,reduce the adverse reactions of drugs to a certain extent,and has been widely used in the process of clinical treatment.In recent years,there are many clinical studies on the combined treatment of IgA nephropathy with traditional Chinese and Western medicine in China.By collecting and summarizing the literature,this paper tries to discuss the combined treatment strategy of traditional Chinese and Western medicine for IgA nephropathy,in order to provide some reference for clinical diagnosis and treatment,and lay a foundation for systematic and in-depth research in the future.
