摘　　要：子痫前期(PE)具备遗传特性、病理特征及发病机制等完全不同方面的两种亚型即早发型子痫前期(EOSP)和晚发型PE。EOSP为妊娠期高血压疾病种类之一,为妊娠期特有、危及母婴健康妊娠的并发症,其具有发病早、病情进展快速等特点,临床表现为妊娠早中期产生快速升高并难以控制的血压和蛋白尿,并伴有严重并发症与靶器官损伤,对母婴生命健康产生严重威胁。此外,EOSP母婴病死率明显高于晚发型PE,且近期预后与远期预后欠佳。故临床需寻找有效的预测方式以早期筛查出可能出现的EOSP高危孕妇,并尽早给予积极干预措施,以降低患病率、控制病情进展、改善母婴预后。本文对现有的EOSP预测方式,如易感基因、母体生物学标志物、特异性miRNA、彩色多普勒超声检测子宫动脉搏动指数(UtAPI)等研究进展进行综述,为临床防治与筛查提供可参考依据。Preeclampsia(PE) has two subtypes with completely different genetic characteristics,pathological characteristics and pathogenesis,namely early-onset preeclampsia(EOSP) and late-onset preeclampsia. EOSP is one of the types of pregnancy induced hypertension diseases. It is a unique complication of pregnancy that endangers the health of mothers and infants. It has the characteristics of early onset and rapid disease progress. Its clinical manifestation is the rapid rise and uncontrollable blood pressure and proteinuria in the early and middle trimester of pregnancy,accompanied by serious complications and target organ damage,which poses a serious threat to the life and health of mothers and infants. In addition,the maternal and infant mortality of EOSP is significantly higher than that of late-onset PE,and the short-term and long-term prognosis are poor. Therefore,it is necessary to find an effective prediction method to screen the high-risk pregnant women with EOSP at an early stage,and give active intervention measures as soon as possible,so as to reduce the prevalence,control the disease progress and improve the prognosis of mothers and infants. This paper reviews the research progress of existing EOSP prediction methods,such as susceptibility genes,maternal biological markers,specific miRNA,color Doppler ultrasound detection of uterine artery pulsatility index(UtAPI),so as to provide a reference basis for clinical prevention and screening.
